Programa de reabilitação multimodal em solo versus exercícios aeróbicos aquáticos sobre o desempenho em testes funcionais de mulheres com fibromialgia: um ensaio clínico randomizado

Abstract

O objetivo deste estudo foi comparar o desempenho em testes funcionais de mulheres com fibromialgia que participaram de um programa de reabilitação multimodal em solo e de um protocolo de exercícios aeróbicos aquáticos. Trata-se de um ensaio clínico randomizado realizado com 49 participantes na Clínica Escola de Fisioterapia da Faculdade de Ciências da Saúde do Trairi (FACISA/UFRN) nos anos de 2024 e 2025. As participantes foram distribuídas de forma aleatória em dois grupos: grupo de exercícios multimodais em solo (GMS) e grupo de exercícios aeróbicos aquáticos (GAA), com duração de doze semanas e frequência de duas sessões por semana. O desempenho funcional foi avaliado por meio dos testes de sentar e levantar de 30 segundos (TSL), Timed Up and Go (TUG), teste de caminhada de 6 minutos (TC6) e dinamometria manual, aplicados em três momentos: antes da intervenção, após seis semanas e ao final de doze semanas. Foi utilizada a ANOVA fatorial com medidas repetidas para investigar os efeitos do tempo (fator intra-sujeitos) e do grupo (fator inter-sujeitos) sobre as medidas nos testes funcionais. Os resultados indicaram uma melhora considerável no desempenho funcional ao longo do tempo para os dois grupos, particularmente nos testes TSL, TUG e TC6, o que sugere um aumento na força muscular, mobilidade e resistência. Embora não tenham sido identificadas diferenças estatisticamente significativas entre os grupos, o protocolo multimodal em solo indicou uma tendência de maior melhora na distância percorrida no TC6, o que sugere um benefício adicional no condicionamento aeróbico. Por outro lado, o programa aquático se mostrou benéfico por diminuir a sobrecarga nas articulações e facilitar a realização dos exercícios com menos desconforto. Ambas as estratégias, o programa multimodal em solo e o protocolo de exercícios aquáticos são eficazes e seguras no tratamento não farmacológico da fibromialgia, resultando em ganhos funcionais e melhoria da qualidade de vida. Os resultados destacam a relevância da prática regular de exercícios supervisionados e indicam a necessidade de mais pesquisas com amostras maiores e acompanhamento mais longo, visando aprimorar a prescrição de exercícios para esse grupo.

Abstract

The aim of this study was to compare the performance in functional tests of women with fibromyalgia who participated in a multimodal land-based rehabilitation program and an aquatic aerobic exercise protocol. This is a randomized clinical trial conducted with 49 participants at the Physiotherapy Clinic of the Faculty of Health Sciences of Trairi (FACISA/UFRN) in 2024 and 2025. Participants were randomly assigned to two groups: a multimodal land-based exercise group (MLE) and an aquatic aerobic exercise group (AAG), with a duration of twelve weeks and a frequency of two sessions per week. Functional performance was assessed using the 30-second sit-to-stand test (TSS), Timed Up and Go test (TUG), 6-minute walk test (6MWT), and hand dynamometry, applied at three time points: before the intervention, after six weeks, and at the end of twelve weeks. A factorial ANOVA with repeated measures was used to investigate the effects of time (in-subject factor) and group (inter-subject factor) on the measures in the functional tests. The results indicated a considerable improvement in functional performance over time for both groups, particularly in the TSL, TUG, and 6MWT tests, suggesting an increase in muscle strength, mobility, and endurance. Although no statistically significant differences were identified between the groups, the multimodal land-based protocol showed a trend toward greater improvement in the distance covered in the 6MWT, suggesting an additional benefit in aerobic conditioning. On the other hand, the aquatic program proved beneficial by reducing stress on the joints and facilitating the performance of exercises with less discomfort. Both strategies, the multimodal land-based program and the aquatic exercise protocol, are effective and safe in the non-pharmacological treatment of fibromyalgia, resulting in functional gains and improved quality of life. The results highlight the importance of regular supervised exercise and indicate the need for further research with larger samples and longer follow-up periods, aiming to improve exercise prescription for this group.
