Saúde mental da pessoa privada de liberdade: itinerário terapêutico em uma Penitenciária Potiguar

Trata-se de uma pesquisa de abordagem qualitativa que busca compreender o itinerário terapêutico em saúde mental de pessoas privadas de liberdade. A pesquisa foi realizada na Penitenciária de Alcaçuz, entre os meses de junho e outubro de 2025, com oito profissionais, a saber: sete profissionais da equipe de saúde e um policial penal que atua na coordenação do setor saúde da referida instituição. Para coleta de dados utilizou-se documentos da gestão do sistema prisional local (relatórios referentes as teleconsultas, registros de vendas de medicamentos, procedimentos de classificação de risco e triagem de pacientes, relatórios à saúde mental e protocolos de recebimento de psicotrópicos), bem como, questionário sociodemográfico e entrevistas semiestruturadas. Os documentos foram submetidos a leituras flutuantes, para identificação da pertinência com relação ao objetivo em tela. Os dados dos questionários sociodemográficos foram analisados à luz de estatística descritiva simples e as falas oriundas das entrevistas foram submetidas a Análise de temática de Minayo. Os resultados foram organizados em quatro seções: I- Perfil da equipe de saúde; II- Perfil das condições de saúde mental dos internos; III- Itinerário terapêutico? /Fluxo de atendimento; e IV- Dificuldades e/ou possibilidades para o atendimento em saúde mental. Conclui-se que a consolidação do itinerário terapêutico no contexto em tela é permeada de dificuldades, desde o próprio contexto da privação de liberdade, a estrutura física das prisões, as carências estruturais e os estigmas que permeiam o direito à saúde da pessoa privada de liberdade. Todavia, iniciativas de organização do fluxo de atendimento e escuta humanizada dos internos têm sido desenvolvidas.

This is a qualitative research study that seeks to understand the therapeutic itinerary in mental health of people deprived of their liberty. The research was conducted at the Alcaçuz Penitentiary, between June and October 2025, with eight professionals: seven professionals from the health team and one correctional officer who works in the coordination of the health sector of the institution. Data collection used documents from the management of the local prison system (reports related to teleconsultations, records of medication sales, risk classification and patient triage procedures, mental health reports, and protocols for receiving psychotropic drugs), as well as a sociodemographic questionnaire and semi-structured interviews. The documents were subjected to open readings to identify those relevant to the objective of the study. The data from the sociodemographic questionnaires were analyzed using simple descriptive statistics, and the statements from the interviews were subjected to Minayo's thematic analysis. The results were organized into four sections: I- Profile of the health team; II- Profile of the mental health conditions of the inmates; III- Therapeutic itinerary/Care flow; and IV- Difficulties and/or possibilities for mental health care. It is concluded that the consolidation of the therapeutic itinerary in this context is fraught with difficulties, ranging from the very context of deprivation of liberty, the physical structure of prisons, structural deficiencies, and the stigmas that permeate the right to health of incarcerated individuals. However, initiatives to organize the care flow and provide humane listening to inmates have been developed.
