Its lower face has a fold of mucous membrane that connects it to the floor of the mouth, called the lingual frenulum. The evaluation of the lingual frenlum in newborns is necessary and should be performed early. Ankyloglossia, known as a prey tongue, is a developmental anomaly characterized by a change in the tongue frenulum that results in limitations of the movements of this structure, which may lead to changes in speech, suction, chew and swallowing. Thus, a description was made of two clinical cases in infants, in both cases the mother reported difficulties in breastfeeding, mainly pain during breastfeeding. Using the protocol of evaluation of the lingual frenlum in the infant, there was a diagnosis of lingual frenulum alteration (case 01) and indication of the frenotomy by performing a detailed description of the surgical technique. In case 02 it was verified that there was no alteration of the lingual frenulum where other aspects that could interfere with breastfeeding were investigated. The diagnosis using specific and validated protocols is of fundamental importance, providing safety in the indication or not of the frenotomy.
