Even with the RDC No. 67 of October 8, 2007, regulating the conditions of infrastructure and manipulation of medicines in hospitals and attributing their responsibility to the pharmacist, many hospitals still support to nursing the practice of transformation of pharmaceuticals forms. Because of the risks and costs associated with inadequate manipulation of medicines for neonates, this study aimed to analyze the transformation profile of solid dosage forms for neonatology in a tertiary maternity hospital, in order to discuss the economic impact and patient safety in the context of RDC No. 67 /2007. This is a cross-sectional, retrospective and analytical study, whose data collection was carried out through the AGHU, containing patients admitted to the UTIN and UCIN during 2018. All prescribed solid dosage forms that were transformed were collected and analyzed their profile and cost based on the Health Price Database (BPS). For comparative purposes, the cost of these extemporaneous preparations in a unitarization room and another one, with the outsourcing of these compoundings, was simulated. Of the 658 newborns, 108 (16.4%) used the transformation of tables according to the prescription summary, accounting total of 9115 doses administered, with the greatest demand attributed to medicines for the treatment of disorders of the cardiovascular system. With the cost simulation with outsourced compoundings and extemporaneous preparations in the unitarization room, the latter was the one that would generate the greatest economic impact on the service (a 57% reduction in outgoing). In addition to economy, the most important factor is patient safety by of the preparation of extemporaneous suspensions centralized in the pharmacy service. Then, the study was able to identify the profile of trasnformation of solid dosage forms in tertiary maternity hospital, and deduce that the implementation of a unitarization room for the manipulation of extemporaneous preparations in neonatology is feasible from an economic and patient safety point of view.Attribution-NonCommercial-NoDerivs 3.0 Brazilhttp://creativecommons.org/licenses/by-nc-nd/3.0/br/Preparações farmacêuticas.
