The objective of this study was to evaluate the accuracy of the probabilistic relationship in the identification of deaths from individuals aged 60 years and over in the state of Rio Grande do Norte, in the years 2010 and 2015. This is a multi-phase epidemiological study encompassing bibliometric review, epidemiological mapping and diagnosis of death by fall in the elderly, linkage of falls death records reported to the Mortality Information System (SIM), and hospitalizations with information of falls in the Hospitalization Information System (SIH) using RecLink software. The reviews did not have a consistent epidemiological outlook and the quality of the data was questioned, as well as the possibility of masking the real underlying cause. In 2010 and 2015, a total of 23631 deaths of the elderly were informed to SIM, and 106 of them were caused by falls. In the same period, the SIH informs 3107 hospitalization procedures of the elderly with primary or secondary diagnosis of falls, of which 163 progressed to death. In 2010, the method presented sensitivity of 99.9%, specificity of 74.7%, positive predictive value of 65.4%, negative predictive value of 99.9% and accuracy of 82.8%. In 2015 these values were: respectively 99.8%, 83.8%, 62.6%, 99.9% and 87.2%. After the linkage the Specific Mortality Ratio per fall was adjusted in 2010 from 15.9 to 65.2 / 100,000hab, and in 2015 from 14.7 to 64.3, an increase of 308% and 333% respectively. These results suggest that the probabilistic linkage between the SIM and SIH / SUS databases contributed positively to the improvement of the quality of information about death by fall in the elderly, allowing a new look at the indicators and the need for actions to avoid mortality from this cause.
