Design: This was a randomized, double-blind, controlled clinical trial. Participants were randomly assigned to two groups: active tDCS (n=20) or sham tDCS (n=17). Setting: The study was conducted at the Neuromuscular Performance Analysis Laboratory of the Federal University of Rio Grande do Norte (UFRN). Participants: The sample consisted of 37 individuals aged between 18 and 60 years with NSCLBP. Intervention: The intervention was performed twice a week for 5 weeks, totaling 10 sessions (20 minutes, 2mA, applied over the dorsolateral prefrontal cortex). Outcome measures: Pain intensity (VAS), pressure pain threshold (PPT - algometry), and disability level (Roland-Morris Disability Questionnaire, Oswestry Disability Index, and sit-tostand test) were assessed pre- and post-5 weeks of intervention. Results: The active tDCS group showed a significant reduction in pain levels compared to the sham group (sham tDCS: from 4.70 ± 2.59 to 3.66 ± 2.55; active tDCS: from 5.40 ± 1.64 to 2.02 ± 1.72; p=0.026). However, there were no significant differences between groups for PPT (L5D sham tDCS: from 4.06 ± 1.78 to 4.18 ± 2.54; active tDCS: from 3.81 ± 2.11 to 4.86 ± 2.61, p=0.427; L5E sham tDCS: from 4.05 ± 1.98 to 5.13 ± 2.54; active tDCS: from 3.85 ± 2.64 to 5.31 ± 2.87, p=0.843; L3R sham tDCS: from 4.33 ± 2.39 to 5.84 ± 3.02; active tDCS: from 3.92 ± 2.70 to 5.40 ± 2.78, p=0.649; L3L sham tDCS: from 4.76 ± 2.73 to 5.73 ± 2.72; active tDCS: from 4.07 ± 2.86 to 6.01 ± 3.34, p=0.788), ODI (sham tDCS: from 24.65 ± 12.18 to 16.47 ± 10.62; active tDCS: from 21.65 ± 11.80 to 11.25 ± 8.93, p=0.113), RMDQ (sham tDCS: from 7.76 ± 4.38 to 4.76 ± 4.18; active tDCS: from 8.95 ± 5.24 to 3.50 ± 2.87, p=0.285), and sit-to-stand test (sham tDCS: from 11.29 ± 2.79 to 9.91 ± 3.38; active tDCS: from 13.51 ± 5.09 to 10.34 ± 3.82, p=0.726).
