It is the most common primary malignant tumor of the bone, accounting for approximately 20% of the sarcomas, but only 5% of the osteosarcomas occur in the jaws. Their aspects so clinical and histological as the course and prognostic are variable. This article shows a case report of a 20-year-old man, that presented swelling near of the mandibular left premolar. After clinical diagnostic of lesion of the dental periapex, the patient was undergone initially the endodontic treatment of the tooth involved. Thereafter, in a period of eleven days, could observe an increased considerable of the lesion resulting in visible facial asymmetry. The occlusal radiographic revealed image compatible with area radiopaque resembling sunray, suggesting diagnostic of osteosarcoma. The treatment comprised partial mandibulectomy and reconstruction of the area, using bone of the rib and graft of skin of the buttock for cover the oral mucosa involved.
