However, there are flaws in the counter-referral of women with syphilis from maternity hospitals to Primary Care. This study aimed to build and validate a counter-referral form for women with gestational syphilis treated at a public maternity hospital. This is a methodological study. In order to reach the proposed objective, field research was carried out in the regional maternity hospital of habitual risk in the metropolitan region of the State of Rio Grande do Norte, an integrative review to support the construction of the form; and finally, the validation of the instrument with the Delphi Technique through the consensus of judges, where a Content Validity Index with a value equal to or greater than 78% was adopted. The study was approved by the Ethics and Research Committee of the Federal University of Rio Grande do Norte through opinion number 4,579,331 and Certificate of Presentation of Ethical Appreciation number 43346321.5.0000.5537. Data were analyzed according to the nature of each study and organized in tables, charts and in a descriptive way. The results of the field research presented a scenario in which syphilis screening is carried out in the maternity ward, however there are flaws in the management of the disease during the woman's hospitalization in the institution. Another finding in this same research is that there is no specific instrument for the counterreferral of cases of gestational syphilis. The integrative review compiled a contribution of 15 scientific articles that supported the construction of the preliminary form. This instrument was validated by 16 judges, maintaining in its structural form a title and seven items that record necessary information about gestational syphilis in the maternity hospital for the follow-up of women to Primary Care. The instrument was validated in a single round with the Delphi Technique with an overall Content Validity Index of 0.99% by the evaluation criteria of clarity, practical pertinence, theoretical relevance, and theoretical dimension. This instrument was presented as a technological product to the Postgraduate Program in Health and Society of the School of Health of the Federal University of Rio Grande do Norte for the conclusion of the Professional Master's Degree in Health and Education Practices.
