Methods: This randomized, double-blind, spli-mouth clinical trial applied PES in 28 subjects with bilateral Milers’s Class I or II gingival recessions undergoing subepithelial connective tissue graft (control) graft root graft surgery and one collagen matrix (test) at baseline and after 6 months. The PES parameters evaluated were: mesial papilla (PM), distal papilla (PD), level and contour of the gingival margin (NMG, CMG), volume, color and texture of the inserted gingiva. Data were statistically analyzed by paired Student's t-test, with a significance level of 5% being pre-established. Results: A low score of 0-5 was obtained two (5%) and one (4%) times for the control and test groups, respectively, in baseline versus two (8%) and one (4%) times after 6 months, for the control and test groups, respectively. The mean score of 5-10 was obtained 23 (88%) and 24 (96%) times at the baseline, and 23 (93%) and 24 (96%) times at the follow-up for the control and test groups respectively. There was a statistically significant difference only in the intragroup analysis, when the mean values of the "distal papilla" variable in the control group (p = 0.004) and the "gingival margin contour" variables (p <0.001) and "gingival margin level" <0.001), in the test group, in the two evaluated periods. Conclusion: It was concluded that the Pink Esthetic Score (PES) is a gingival aesthetic analysis tool that directly influences aesthetic perception by the professional.
