This case report describes the treatment of a 21-year-old female patient with bilateral agenesis of upper premolars and one missing lower left premolar. Orthodontic treatment began at age 13 due to Class II malocclusion. At 21, the patient underwent dental implant placement, which later required removal. A new treatment plan was developed, including additional orthodontics, gingivoplasty of teeth 12 to 22, bone grafting with biomaterials, and placement of new short implants bilaterally in the premolar region, with metal-ceramic prostheses. The patient reported satisfaction with the outcome, which provided improved self-esteem and in the quality of oral health. This case reinforces the importance of interdisciplinary planning in implantology, emphasizing the need to ensure adequate prosthetic space, careful management of peri-implant tissues, and periodic follow-up.
