In this context, modernizing and revolutionizing this area of Dentistry even further, the Guided Endodontics technique emerged in 2016, bringing more safety and predictability to the treatment of calcified root canal systems, which were previously seen as highly complex due to the difficulty in locating the canals, and with high risks of iatrogenic accidents. Objective: Clinical case report of calcification of the root canal of a 45-tooth tooth using the Guided Endodontics technique, verifying its applicability, advantages and limitations. Case Report: A 27-year-old female patient attended the Emergency Room of the Dentistry Department of the Federal University of Rio Grande do Norte in 2023, reporting as her main complaint pain caused and located in tooth element 45 that presented calcification of the root canal, confirmed by periapical radiography and with the aid of a cone beam computed tomography (CBCT). After studying the possible treatment routes and knowing that guided endodontics is a treatment proposal for teeth with root calcification, this innovative technique was chosen. The treatment was performed by performing guided endodontic surgery using a rotary motor and a 1.4 mm diameter drill. Conventional endodontic treatment and cavity sealing with Llis composite resin were continued.
