In Brazil, this is one of the main mortality causes, it may be a reflex of the current population pattern resulting from demographic changes, with high prevalence and low control rates, regarding the adult population. Bearing this in mind, one needs to have a larger emphasis on actions of control, prevention and early detection with respect to the cases, with the goal of reducing complications. Thus, there is a need for greater emphasis on the actions of control, prevention and early detection of cases in order to reduce complications. In this sense, self-care allows the user to become a modifying agent of their health condition by understanding some factors that involve the disease. The objective of this study is identify the self-care actions accomplished by the users with diabetes mellitus on the primary health attention field. Type study descriptive, exploratory study with a quantitative approach, carried out from October 2017 to April 2018, on the Basic Health Units, with the family health strategy care model, on North and South’s health district in the city of Natal. The stratified random sample, with reliability interval of 95% and error margin of 5%, calculated is from 379 diabetic patients, and a total of 387 users searched. The subjects with diabetes from 18 years old will take part on the study, registered by the researched units through the signature of a Term of free and clarified consent in accordance with Resolution 466/12 from the National Health Council. Two research instruments were used used: one with socioeconomic variables and another, guided toward the patients’ self-care activities. Descriptive statistical analysis was used, with mean, median, standard deviation and the parametric Student T test for a significance level of 5%, in the comparison of the PAF by region. Data were analyzed using SPSS software, version 20.0. The results by region of study indicated that the majority of respondents (91.73%) were from the North district; the predominant age group was older than 60 years (53.23%); in addition to prevailing: female (71.06%), married (49.87%), with more than five years of diagnosis of diabetes (53.75%), which did not attend a diabetes support meeting (90.44% ) because there was no meeting at the health unit (32.29%). The majority (50.13%) claimed to know about self-care and carried out actions self-care with diabetes (68.22%). Regarding the results of diabetes self-care based on seven days of the week, medication-related items had a greater number of days of actions performed (6.39), while actions related to physical activity (1.10) and blood glucose control (0.97) were the least executed actions. When compared to the health districts surveyed, there is evidence of statistical difference between users; it was observed that the diabetics from the south showed a greater number of days in relation to following a healthy diet (4.94, value -p: 0.010), ingesting red meat and / or derived of the whole milk (3.34, p: 0.044) and performing physical activities for at least 30 minutes (2.56; p-value: 0.048). The identification of the actions self-care by the patients with the disease and their main difficulties, as well as the characterization of the health units of the primary health care to the actions carried out with these users, allowed to identify the way in which self-care was developed and the need for assistance protocols focused at self-care of the service user with the disease.
