In this scenario, patient safety stands out as an essential dimension of quality care, requiring coordinated and structured actions for risk management. For these actions to be effective, it is essential that people and structures are well involved, trained, documented, and supported by adequate organizational mechanisms at all points of health care. In Brazil, the National Patient Safety Program, established in 2013, is a strategic initiative aimed at promoting quality care and reducing preventable adverse events in health services. Internationally, the Global Action Plan for Patient Safety 2021-2030 reinforces the importance of global efforts to minimize preventable harm, aligning international guidelines with local needs and challenges. However, despite the established guidelines, the consolidation of effective patient safety practices still faces difficulties, especially with regard to the structuring of Patient Safety Centers, which are the bodies legally responsible for implementing actions related to safety in health services. In this context, strengthening the NSP is an urgent need to ensure continuous improvement in the quality of care and risk mitigation. Objective: This project aims to improve the quality of Patient Safety Centers in state hospitals, promoting the implementation of organizational practices that enhance care safety. Method: To this end, a quality improvement analysis study was conducted, of the action research type, in the NSP of state hospitals. Results: The results demonstrated a significant improvement in the criteria for document consolidation of the Patient Safety Centers, with advances in the organization and systematization of information related to patient safety. One of the main contributions of the study was the development of a parameterization spreadsheet that allows a broad evaluation of the NSP's documentary framework, promoting greater management efficiency. In addition, an innovative information system was developed that integrates data in an accessible, reliable, and up-to-date manner, facilitating decision-making in both central management and hospital services. Conclusion: The direct impact of these actions is reflected in the improvement of the quality of care provided to users of the Unified Health System, promoting more efficient risk management and reducing the occurrence of adverse events.
