There are currently 7.1 million cases worldwide, being prevalent in the reproductive age group of 15 to 49 years. These indicators motivated the World Health Organization to develop intervention strategies for the adequate management of sexually transmitted infections, by the year 2030. Therefore, this research aims to develop the online course plan for adult health education on syphilis in Primary Health Care. This is a methodological study with a quantitative approach, based on four stages based on the contextualized instructional design framework proposed by Filatro: analysis, design and development, implementation and evaluation. In the first stage, a scoping review was carried out to map knowledge about syphilis among users treated in primary health care and relate it to Orem's Self-Care Deficit Theory, between November and December 2022; subsequently, a cross-sectional study was carried out with 269 users served in the city of Natal, Rio Grande do Norte, to identify the knowledge of the adult population about syphilis, in primary health care, between April and May 2023. The data was analyzed using descriptive and inferential statistics, with the Chi-square and Fisher tests, evaluating significance at p ≤ 0.05; in the second stage, the thematic axes for the content of the online course plan were structured, containing the objectives, contents, methodological path, evaluation and curricular structure; in the third stage, educational resources were configured, based on training tools, ambience, teaching-learning situation, modules and assessment; In the evaluation stage, content validation was carried out with experts in the nursing field, recruited via the Lattes platform and the snowball strategy, scored according to the criteria for selecting judges established by Fehring, with a minimum cutoff point of 4 points. The final sample included 11 experts. The data collection instrument included the sociodemographic characterization of the study participants, in addition to questions aimed at the user's knowledge about sexually transmitted infections, mainly syphilis. Descriptive and inferential statistics were performed for Kappa values > 0.6 and CVI > 0.8, including adjustments to items, information, content and grammar. The research was approved by the Research Ethics Committee of the Federal University of Rio Grande do Norte, complying with the ethical precepts of Resolution No. 466/12, from the National Health Council. The results of the scoping review demonstrated gaps in the knowledge of syphilis regarding clinical manifestations, deficits in prevention, testing, diagnosis and implemented therapeutic practices, categorized based on the self-care deficit theory from Orem. In the cross-sectional study, regarding sociodemographic variables, users were identified as being over 30 years old (148; 55.02%), a higher percentage of women (214; 79.30%), single (127; 47.21) and completed high school (112; 41.64%). Regarding knowledge about syphilis, users' understanding of the definition of the disease stands out (238; 88.48%), in addition to participation in health education actions through educational resources (163; 60.59%). The analysis and relationship between the variables demonstrated important results, such as the definition of syphilis related to the service area (p=0.004), marital status (p=0.001), religion (p=0.026) and education (p=0.008). Regarding the creation of an online course on syphilis, a significant relationship was found with education (p=0.013). Based on the results and analysis, the online course plan instrument was created, consisting of four modules: sexually transmitted infections and their social impact; clinical aspects of syphilis; limitations to self-care; effective self-care and the contribution of technologies. The online course instrument was validated for 84 items relating to content, in a Delphi round carried out with 11 experts. Kappa values > 0.6 and CVI > 0.8 were considered for the criteria of language clarity, practical relevance, theoretical relevance and theoretical dimension, reaching satisfactory rates for 80 items; in the end, item 04 was excluded from the general aspects; adjustments to items 48 and 50, of module II, and item 59 of module III, for the clarity criterion. Given the results obtained, the items could be kept in the instrument and validated with just one Delphi round.
