Method: This is a Scoping Review based on Joana's instructions Briggs Institute. Searches in the databases took place from February to May de 2023. Publications were selected that responded to the objective of the study and that were available in full from. Results: The final sample had seven articles, with a predominance of publications from the year 2022 and Switzerland as country of origin. The multidisciplinary team stood out for being the responsible for the transition care of the patient with Lateral Sclerosis Amyotrophic for the home. Regarding care, the studies addressed assistance to the basic needs of life, planning, creating protocols, using telehealth and communication between services. Conclusions: Thus, care for transition of the individual with Sclerosis Lateral Amyotrofic to the home range from assistance to basic human needs to articulation with other health services.
