Within the framework of Specialized Care, the city of Caicó/RN offers the Specialized Centre in Rehabilitation III (CER-III) as one of the care components of the network, with the participation of various professionals and collaborative institutions, for example the program of Multiprofessional Residence in Maternal and Child Health. Through the context of care beyond the patient and considering the overload and psychological suffering of the relatives and/or caregivers was created the service of Psychological Duty for patients responsible of CER-III which is proposed to generate psychological support in emergency character. As a form of structuring the Psychological Duty service and to be used by it, this work consisted in the elaboration of a psychological records instrument directed to the service in question and built from the contact with other similar services and a review of the literature.
