In this context, experience with rural medicine becomes something essential in the training of the future family doctor, given the need to offer an adequate service for the population of these areas and in accordance with the general contexts experienced by them. Objective: To propose the creation of an internship in rural medicine in the residency program in Family and Community Medicine at a federal educational institution in the sparsely populated areas of Rio Grande do Norte. Methodology: The project was built in two stages: 1) an integrative revision and; 2) elaboration of a product. After performing the first stage, only 04 articles were listed, considering the theme involved. To provide more support for the construction of the proposal, gray literature and consultation with experts who have had experience with the theme were used. From then onwards, the syllabus for the internship in rural medicine was drawn up. Results: the internship in rural medicine will be part of the theoretical-practical modality, lasting 30 days and being mandatory, with a workload of 160 hours. The practical activities will be performed in the Family Health Team in which the resident will be allocated. Theoretical activities will follow the schedule defined by the coordinator of the residency program. Conclusion: The external internships are essential in the training of residents, especially the rural medicine internship, mainly considering the context of cities in the sparsely populated areas of the state of Rio Grande do Norte, formed by a significant portion of the rural population.
