This narrative review article analyzes the historical and political milestones that structure Persons with Disabilities access to primary health care, as well as the barriers still present in this context. The research is based on ministerial documents and scientific literature, identifying advances and challenges in the implementation of inclusive health policies. Eleven publications were identified as part of the final sample, located through searches on the Google Scholar platform 7 and official websites of the Federal Government, using the descriptors: "Persons with Disabilities," "Primary Health Care," and "Access to Primary Care." The analysis of the results identified that despite normative advances such as the National Health Policy for Persons with Disabilities and the Network of Care for Persons with Disabilities, there are still structural, linguistic, and behavioral barriers compromising the effectiveness of care. Basic Health Units are often inaccessible, and healthcare professionals frequently lack inclusive communication skills, such as the use of Brazilian Sign Language. Additionally, discriminatory and stigmatizing attitudes still exist, negatively impacting the quality of care provided to Persons with Disabilities. It is concluded that overcoming these barriers requires continuous investment in professional training, accessible infrastructure, and the promotion of a culture of inclusion and equity in the healthcare system.
