Objective: to identify the main drug interactions and medications implicated in pre-renal transplant patients. Methodology: cross-sectional and multicenter study in three capitals of northeastern Brazil between July 2019 and January 2020 (thirteen institutions involved). A total of 1,153 individuals who were on the kidney transplant waiting list of both sexes were included. Interactions were classified according to severity using the Micromedex © database. Data were analyzed descriptively using Stata © 15 software. Results: the main causes of chronic kidney disease were systemic arterial hypertension (47.5%) and diabetes mellitus (18.9%) and the prevalence of one or more interactions was 52.4% in patients, with 21.4% of severe interactions. The most commonly involved pharmacological classes were antihypertensives (calcium channel blockers and adrenergic agonists) and statins (simvastatin). The main risks involved in these interactions are hypotension, bradycardia, and toxicity. We can also observe that acetylsalicylic acid is potentially responsible for the risk of interactions in the study patients, bringing risks such as hypotension, hypoglycemia, and hemorrhages.
