Among these errors, prescription errors stand out, which can not only reduce the expected therapeutic efficacy, but also present risks to the patient’s health. The objective of this study is to promote a qualitative and quantitative analysis of the pharmaceutical occurrences reported in the Prescription Evaluation Center against prescription errors that occur in a teaching hospital (Hospital Universitário Onofre Lopes). Data collection was done digitally, using the spreadsheet named “produtividade de amarelinhos”, in which the pharmaceutical interventions performed are recorded. An analysis and construction of graphs and tables was carried out through Excel, making it possible to analyze the results. A total of 1490 drug-related problems were identified, with an average of 124 records per month, 41% of which were related to elderly patients. Excluding problems related to antimicrobial control, the main drug-related problems found were high dosage (26,63%), incomplete prescription (20,37%) and inadequate dilution (17,17%). Among the high-risk medications, those most associated with errors were glucose 50% (115), opioid analgesics (37) and insulins (22). Some drug-related problems that were identified present a higher risk potential for elderly patients, such as high doses of opioids, that could provide an increased risk of respiratory depression. It is concluded that several drug-related problems identified can cause severe impairmet to the patient’s health status, potentially impacting their length of stay and costs to the hospital, so extreme caution should be required during prescription and clinical monitoring.
