Therefore, identifying the functional status of these individuals is crucial for a good prognosis. The research aimed to identify the functional trajectory of patients admitted to an ICU of a Teaching Hospital of northeastern Brazil. This is a retrospective observational cohort study, which analyzed data from patients admitted between January and December 2023 to the adult ICU of the Hospital Universitário Onofre Lopes, in Natal/RN. Demographic variables, level of mobility using the JH-HLM scale and its variation were collected. The data were extracted from monitoring forms used by the hospital's physiotherapy team and tabulated in the Excel program. Continuous variables between the “Worsening” and “Maintenance” groups were significantly correlated at p < 0.05. At the end of the study, 578 patients were included in the analysis, with a mean age of 58.6 ± 15.4 years, 56.2% were male, 4.7 ± 8.0 days of hospitalization and with a level of pre-admission mobility of 8.0 ± 0.0, at discharge 5.0 ± 2.4. In the functional track, 72.5% showed worsening, required more time for the 1º standing position and remained in intensive care for longer. In conclusion, ICU admission promotes functional decline in previously independent patients.
