For data collection, the nurse needs an instrument that will serve as a guide and that reproduces the needs of the person being cared for. The objective of this study was to build and validate a Nursing History instrument for a Pediatric Intensive Care Unit of a University Hospital. It is a methodological study, with a quantitative approach that used Wanda Horta's Theory of Basic Human Needs as a theoretical framework and Pasquali (2010) as a methodological framework, which emphasizes the importance of the instrument validation process. The study it was be developed from three poles or stages: the first consisted of the Theoretical polo, with a scoping review in order to select the results to compose the construction items of the instrument for Nursing History, guided by the framework of the Theory of Basic Human Needs by Wanda Horta and construct elaboration; the second, consists of empirical procedures, was based on the judgment of the content and appearance of the construct by 15 judges, experts in the field of Nursing, using a Likert-type scale and semantic evaluation of items and domains through the Brainstorming technique; the third consists of statistical procedures with statistical analysis and validation of the final version of the instrument through the content validity index (CVI) and Kappa concordance coefficient (k). As a result, six studies were obtained in the first pole, which made up the final sample of the scoping review and through which the first version of the instrument was organized, with seven domains and 78 items. In the second pole, the Judges and experts validated the content and appearance using a Delphi technique round, where two items obtained an agreement index below the cutoff point (<0.64). The instrument did not change after the semantic assessment of the items and domains using the Brainstorming technique, since the participants demonstrated understanding of all items, with agreement above the cutoff point (>0.70) for all items. At the end of the evaluation, the final instrument contains seven domains and 79 validated and understandable items to be used in the Pediatric ICU.
