In 2021, Brazil recorded ten maternal deaths per day and estimates of one fetal death every 27 minutes. The World Health Organization developed the Safe Childbirth Checklist as a tool to remember actions that, if forgotten, may pose a risk to safety in childbirth care, and recommends the use of the Safe Childbirth Checklist, adapted to local contexts. Objective: Carry out the adaptation of the Safe Childbirth Checklist to the context of four Brazilian maternity hospitals. Methods: Methodological study of adaptation of the basic Checklist, using a mixed consensus technique with the participation of judges from multidisciplinary teams who worked at the four moments of application (admission, pre-delivery, delivery and discharge), carried out in four hospital units with obstetric services in the states of Rio Grande do Norte and Rio de Janeiro.To validate the items, a Content Validity Index with a cutoff point of 90% and at least two independent and secret votes were considered. The study was carried out between October 2022 and May 2023, after approval by the Research Ethics Committee of the Hospital Universitário Onofre Lopes, Universidade Federal do Rio Grande do Norte. Results: A second version of the Brazilian Safe Childbirth Checklist was obtained with 42 items and four specific versions for each of the services, with the inclusion of relevant items such as rapid testing for HIV, syphilis and hepatitis, updating of parameters for the use of antiretrovirals and the E-MOTIVE bundle for preventing postpartum hemorrhage. The item on episiotomy was excluded from all versions.
