Fistulas arise after the formation of epithelium between the cavities, resulting from an untreated oroantral communication. It is known that communication is caused by trauma and surgical procedures, such as: extraction of dental elements, lifting of the maxillary sinus and removal of cysts and tumors in the maxilla. The clinical diagnosis is based on the patient's symptoms, such as: nasal congestion and passage of liquid into the nasal cavity when eating; clinical maneuvers such as Valsalva and imaging tests such as computed tomography. Objective: To study a case of oral-sinus fistula closure using a buccal fat pad, evaluating its advantages over other techniques used. Case study: Patient with bucosinusal fistula arising from complications in the transoperative period of maxillary sinus lifting surgery. After four failed closure attempts using a repositioned palate flap, it was decided to perform a fistulectomy with occlusion of the defect through the buccal adipose body. The closure of the defect with the buccal adipose body was performed uneventfully and brought resolution to the case, after thirty days the suture was removed and after ninety days, the entire area was healed.
