The sample consisted of 54 cases of OLs diagnosed in a period of 10 years, of which demographic and clinical data from medical records were collected. Analysis of histological grading of cases submitted to biopsy and clinical follow-up was performed. Statistical tests were performed adopting a significance level of 5% (p ≤ 0.05). Of the total sample, the majority were female (63%) and the mean age of patients was 58.93 years. Dark brown or black type of skin (61,1%) was predominant and the vast majority reported being smokers or former smokers (81.5%). Half of the patients had multiple lesions in more than one location (50%). The oral mucosa was the more committed site (30.3%) with predominance of homogeneous leukoplakia (70.8%). Biopsies were performed in 35 cases considering the histological grading the binary system the majority of cases were diagnosed as lesion with low risk of malignant transformation (34.4%). Only 18 patients returned for clinical follow-up evaluation. The follow-up period ranged from 3 to 96 months and 11 patients (61.1%) showed no recurrence. There were no statistically significant differences between the variables and histopathologic gradations of malignancy.
