This study set out to develop an instrument for the nursing consultation, which complies with legal standards and the taxonomic system adopted by the institution, to be used in both physical and electronic medical records. The methodological framework adopted was Convergent Care Research with a qualitative approach. The setting was the Ophthalmology Unit of the Cassiano de Moraes University Hospital, with the participation of 10 professional nurses selected for convenience. Data was collected using participant observation and convergence group techniques, after the research had been approved by the Ethics Committee and the institution's Letter of Consent. As for the theoretical framework, it was identified from the research scenario, the theory of “Basic Human Needs”. The process of constructing the phases of the Convergent Care Research made it possible to understand the role of nursing in an ophthalmology unit, highlighting aspects related to the Nursing Process, the difficulties and challenges faced, and the structuring of the Nursing Consultation for preoperative patients. The results were expressed as products aligned with the objectives of the study, such as a semi-structured script and information aimed at guiding these patients, and the lack of validation of these instruments represents a limitation of the study. Despite this restriction, the study shows that strengthening planning and overcoming barriers are fundamental to consolidating the nursing consultation as a systematized and personalized practice.
