Mandibular exostosis is usually asymptomatic, except in cases of secondary trauma, therefore, they usually do not require surgical treatment. Objective: The present study aimed to report the removal of bilateral mandibular torus in the mandible of a patient attending the oral surgery service of the Federal University of Rio Grande do Norte. Case Report: A 29- years-old male patient, attended at Clinic I of the Dentistry Department of the Federal University of Rio Grande Do Norte, with a main complaint of volumetric increase in the lingual region of the mandible. He reported an increase in the volume of the region over the years and frequent episodes of local trauma and difficulty in speaking and swallowing certain foods. In the extraoral and intraoral exams, there was a bone growth with a nodular aspect, hardened consistency and covered by oral mucosa in the entire region of the lingual floor of the mandible, in addition to reddened areas possibly associated with trauma. The radiographic examination revealed the presence of mandibular exostosis bilaterally, and the clinical-radiographic diagnosis of mandibular torus was performed. The surgical removal procedure of the torus was performed and the patient is currently being followed up, showing a satisfactory healing process. Conclusion: Removal of the bilateral mandibular torus is essential in cases where exostosis affects the patient's health, thus avoiding future trauma episodes and phonetic and swallowing difficulties.
