However, both acquired syphilis and congenital syphilis have increased dramatically in the last two decades in Brazil. In 2019, the detection rate of syphilis in pregnant women was 20.8/1,000 live births and the incidence rate of congenital syphilis, 8.2/1,000 live births. Throughout the national territory, compulsory notification of congenital syphilis, syphilis in pregnant women and acquired syphilis began in 1986, 2005 and 2010, respectively. The numbers of cases of infection are worrying, despite all the efforts of recent years. The World Health Organization's goal is to reduce congenital syphilis to 0.5 per 1,000 live births. Objective: To develop a collaborative proposal using quality improvement tools for the prevention of maternal-fetal syphilis. Methodology: It is about the elaboration of an execution plan for the prevention of maternal-fetal syphilis using the improvement model, creating a theory of change and a measurement strategy through the use of indicators. This plan was built by a multidisciplinary team from the Federal University of Rio Grande do Norte (UFRN) and the Institute for Healthcare Improvement (IHI), under the aegis of the IHI Breakthrough Series Collaborative methodology. A collaborative pilot study is underway, in which improvement teams from Basic Health Units in 10 municipalities in Brazil are testing the proposed theory. The measurement strategy, built to evaluate the impact of the theory of change in pursuit of the objective, was built with the aim of monitoring the main processes involved, having determined result, process and balance indicators. The approach to achieving improvement in a collaborative project is based on testing and learning: develop a changing idea, find a way to test it, and observe how the system will react to this change over time, based on tracking data. Results: The Guiding Diagram was organized into five primary drivers, which directly impact the project's objective: 1) Pregnant women with access to prenatal care; 2) Care flow for pregnant women with syphilis; 3) Highly qualified teams; 4) Pregnant women, family members and sexual partners involved and 5) Engaged leaders. These primary drivers are related to eight secondary drivers, subsections of the first ones and which, in turn, are aligned with the change ideas to be tested and will be linked to the indicators. The Measurement Strategy describes, through operational definitions, 12 indicators related to the objectives of the pilot project to be measured by the participants of the Collaborative.
