This was a qualitative study from the perspective of philosophical hermeneutics, carried out in the pediatric ward of the Onofre Lopes University Hospital. The sample size was determined by the theoretical saturation of responses. Six nurses took part in an individual, face-to-face interview guided by a semi-structured script about the phenomenon of hospital discharge in that institution. The results were classified into three categories: the multiprofessional nature of hospital discharge, the (re)insertion of the patient into the Health Care Networks and the systematization of this process by the nurses. A model nursing report for discharging patients from hospital was created based on the interviewees' responses. The group was homogeneous about the importance of a discharge document issued by the profession, the items that would make up this probable document and their apprehension about the insertion of yet another responsibility into their work routine. We suggest validating the document with these same professionals so that it becomes a tool to be implemented.
