Trabalho de Conclusão de Curso (Graduação em Farmácia) - Departamento de Análises Clínicas e Toxicológicas, Universidade Federal do Rio Grande do Norte, Natal, 2024.https://repositorio.ufrn.br/handle/123456789/61295This study aims to evaluate the impact of the role of the pharmacist in the pharmacotherapeutic monitoring of elderly patients with chronic noncommunicable diseases (NCDs), specifically systemic arterial hypertension (SAH) and type 2 diabetes mellitus (T2DM), living in the community of Brasília Teimosa, Natal-RN. The methodology adopted was the SOAP (Subjective, Objective, Analysis and Care Plan) clinical method, adapted to the time conditions and available resources. This is a longitudinal, descriptive and exploratory study, of the case series report type. The target population included three elderly individuals (n=3) aged 60 or over, diagnosed with SAH and/or T2DM, in polypharmacy and with difficulties in adhering to pharmacotherapeutic treatment. Clinical, social and pharmacological data were collected, complemented by laboratory tests. The analysis allowed the identification of drug-related problems (DRPs), classified according to need, effectiveness and safety, culminating in the elaboration of individualized care plans. The results demonstrated that pharmaceutical monitoring contributed to the identification and resolution of DRPs, favoring treatment adherence and control of chronic conditions. Patient 1 showed improvements in glycemic management and adherence to guidelines, but maintained inappropriate use of medications. Patient 2 made progress in glycemic monitoring with family support, but difficulties in adhering to the diet plan persisted. Patient 3 discontinued inappropriate use of medications, but maintained poor glycemic control and resistance to full therapeutic adherence.
