Objective: to define a specific pharmacotherapeutic follow-up tool for the care of high-risk pregnant women. Methods: a descriptive observational study developed at a Brazilian school maternity hospital based on drug prescriptions between January and December 2018. Formulation and its frequency of prescription, the main parameters for monitoring effectiveness and safety were listed. The parameters were incorporated with other relevant data in a structured form to facilitate the investigation of the relationship between changes potentially related to pharmacotherapy. Results: from 6,289 prescribed items, it was possible to characterize the profile of prescriptions, with a predominance of metronidazole ointment (42.8%) and antihypertensives (13.8%). Respiratory changes (9.7% of explained), gastrointestinal disorders (8.6%) and reports of abdominal or pelvic pain (7.8%) were the most relevant assessment parameters. The tool was structured in four elements: clinical data, daily prescription, monitoring parameters and drug-related problems.
