The research took place in two stages: definition and operationalization of the constructs, and theoretical and semantic analysis. In the first step, the scale items were constructed from the Roy Adaptation Model, two integrative reviews of the literature and the interview to people with venous ulcer. In the second stage, 24 judges were selected to judge the appropriateness of the items, which underwent a lexical and grammatical correction, and a pilot test was then carried out to evaluate the understanding of the items by people with venous ulcer. This stage was completed with a new submission to the judges. The theoretical validation process of the instrument followed the Pasquali reference and adopted the Delphi technique for its operationalization. The item that presented a Content Validity Index (CVI) ≥ 0.9 was considered valid. The present study obtained a favorable opinion from the Research Ethics Committee of the Federal University of Rio Grande do Norte, through CAAE nº 65941417.8.0000.5537 and 65941817.1.0000.5537. The first step resulted in the construction of the preliminary items of the instrument, with a total of 76 items divided into Roy's four adaptive modes, 28 items in the physiological-physiological mode, 16 in the self-conceptidentity mode, 21 in the role function mode and 11 in the role function mode. interdependence mode. In the second stage, 14 judges participated in the Delphi 1 round. The percentage of the items that presented CVI ≥ 0.9, in each mode were: physical-physiological 78%; self-conceptidentity 81.2%; of paper function 61.9%; and 100% interdependence. At the end of this step, 17 items were removed from the instrument, 11 were unified, and 3 were reallocated between modes. After performing the lexical and grammatical correction, the scale was submitted to the pilot test and presented CVI ≥ 0.9 in all items, except item 46, which presented CVI = 0.8 but was not removed but modified. Finally, the Delphi 2 round was held with the participation of 13 judges and the scale obtained a CVI ≥ 0.9 in all items. We present the Venous Ulcer Adaptation Scale with theoretical validity, mean CVI = 0.99, with 48 items in total, 17 in physical-physiological mode, 14 in self-concept-identity mode, 4 in function mode paper and 5 in interdependence mode.
