According to the professionals, the KT occurred from the transfer of information during individual consultations. Article 3 aimed to report the experience of translating and disseminating knowledge through the use of digital communication tools in the care of chronic non-communicable diseases in PHC. To perform the KT, official databases regarding CNCDs were consulted. For the dissemination of knowledge, infographics, informational cards, videos, podcast and lay summaries were produced aimed at the target audience and disseminated on digital platforms and radio stations in the Trairi and Potengi region. The use of digital communication tools enabled a greater reach of the target audience, helping to promote health and facilitate access to information on CNCDs. Finally, the findings of the present study indicate that KT is an important tool that can favor the achievement of improvements in health services, through the dissemination of knowledge obtained in research and translated in a clear and understandable way, enabling the development of effective actions and help with decision making.
