In the northeast region, these diseases represent the second leading cause of mortality. In view of this, the present study aimed to analyze the epidemiological profile of respiratory diseases in children living in the municipality of Santa Cruz - RN, during their first year of life. Furthermore, we sought to estimate the incidence of these conditions and identify individual, physiological, behavioral and environmental factors associated. This qualitative and quantitative longitudinal study was carried out in the city of Santa Cruz-RN, from December/2017 to October/2020, and its target was children who were born at the Ana Bezerra University Hospital and their respective mothers. A total of 81 children/mothers who were followed up from 0 to 12 months of life participated in this study through quarterly visits at home (with the pandemic, the interviews became by telephone). For data collection, a structured questionnaire was used for this research containing variables related to socioeconomic, environmental, physiological and behavioral data of mothers and children. For data analysis, the STATA program (version 10.0) was used with assignment of Pearson's correlation tests and the chi-square test, being considered statistically significant values ​​when lower than 5% (p ≤ 0.05). The analysis of the results showed a balance in the sample with regard to sex, where 41 were boys and 40 were girls. The incidence of conditions in the 1st month of the children's life was (36) 44.4%, and this incidence increased in the 4th month, where 55 (67.9%) of them were affected. In the 8th month, the incidence was equivalent to the 1st month, that is, 36 (44.4%) children developed respiratory disease. In the 12th month, the incidence reduced, and 47 (58.02%) children were identified with the condition. The factors associated with the incidence were: maternal age (p=0.006), level of education (p=0.045), family income (p=0.038), interruption of breastfeeding in the 4th month of the child's life (p=0.034), history of maternal respiratory allergy (p=0.030), trees on the street (p=0.005) and tree in front of the house (p=0.000), type of wall in the house (p=0.005), sharing a room with the child (p=0.018), number of siblings (p=0.048), acquisition of a pet (p=0.037), number of prenatal consultations (p=0.037), use of oxygen support (p=0.046) and hospitalization soon after birth (p=0.033).
