Currently, with the significant advance of highly effective antiretroviral treatment, the profile has changed from a terminal disease to a chronic disease, increasing the life expectancy and quality of life of HIVpositive patients. However, this immune response and maintenance of the viral load until it is undetectable depends on good patient compliance with antiretroviral medications. Aims: To verify the prevalence of adherence to HAART and associated factors in patients on treatment for HIV/AIDS enrolled in the Specialized Care service between January 2019 and December 2021. Methods: Cross-sectional study, with sociodemographic, clinical, and laboratory variables, using medical records, the Logistic Medicament Control System (SICLOM) and the Laboratory Examination Control System (SISCEL) as a source of data. Results: Of the 100 patients studied, 61% reported no adherence difficulties, 44 individuals had CD4 levels between 501 and 1,000 cells, and approximately, 25% viral load detection. 39 subjects had opportunistic infections. The majority were male (77%) and single (58.3%), with complete high school education (34%) and resided in the municipality of Caicó (32%), with more than half being employed and with an income of up to 1 minimum wage, and the predominant labor occupations: Household; ASG and farmer. Most (67.7%) reported fixed partner, know about safe sex and shared the diagnosis with someone. The variables schooling, opportunistic infections, CD4 lymphocyte levels, and viral load were associated with difficult adherence.
