    The article discusses the urbanization of visceral leishmaniasis in Brazil, which was previously prevalent in rural areas of the Northeast but now also affects peri-urban areas in other regions such as the Southeast and Central-West. The lack of effective control of the vector and reservoir complicates disease management. Intervention measures, such as culling infected dogs and insecticide spraying, are implemented late, after human cases are identified, due to the long incubation period of Leishmania. Studies demonstrate a high rate of asymptomatic infection in humans and dogs, suggesting the need to better understand the role of this infection in transmission. The adaptation of the vector, Lutzomyia longipalpis, to peri-urban environments expands endemic areas. Control measures include community education, identification of risk areas, and decentralization of epidemiological surveillance. The article highlights the need for new control strategies and collaboration between the Ministry of Health and the Brazilian scientific community
