(Universidade Federal do Rio Grande do Norte, 2025-07-03) Alves, Ângelo Felipe Lima de Araújo; Dantas, Gabriel Diniz Câmara; Oliveira, Luanny Vitoria da Silva; Lisboa, Fernando Freire; http://lattes.cnpq.br/1660009081537484; 0009-0005-9202-0539; http://lattes.cnpq.br/2373176978285019; Quinino, Reynaldo Martins e; Medeiros, Auzelivia Pastora Rego
Introduction: Gastroesophageal Reflux Disease (GERD) is a chronic condition with a high global prevalence that significantly impairs patients’ quality of life. When clinical therapy fails or complications arise, surgical treatment—particularly Nissen and Toupet fundoplications—emerges as an effective option. Objectives: To compare the Nissen and Toupet fundoplication techniques in the surgical management of GERD, evaluating their efficacy, complication rates, and impact on patients' quality of life. Methods: A systematic literature review was conducted using the PubMed, Cochrane, SciELO, and LILACS databases, including studies published between 2002 and 2025. Randomized clinical trials and cohort studies directly comparing both techniques with a minimum follow-up of 12 months were included. Results: Nine studies were included, encompassing a total of 2,130 patients. Both techniques demonstrated comparable efficacy in symptom control and pH-metry normalization. Nissen fundoplication was associated with a greater increase in lower esophageal sphincter pressure but also a higher incidence of postoperative dysphagia in the immediate period. Toupet fundoplication showed a lower rate of dysphagia and better preservation of esophageal motility. In the long term, outcomes related to proton pump inhibitor use, quality of life, and patient satisfaction were similar between the two techniques. Conclusion: Nissen and Toupet fundoplications are effective surgical options for GERD treatment. The choice between techniques should be individualized, considering each patient's clinical and functional characteristics, particularly based on preoperative assessment of esophageal motility.

Introduction: Genitourinary syndrome of menopause (GSM) significantly impacts breast cancer survivors (BCS). Physical methods, such as laser and radiofrequency, have shown promise as treatment options, making it essential to analyze the scientific literature for their application in this specific group. Methods: This systematic review followed the PRISMA protocol registered with PROSPERO (CRD42023387680). An extensive database search was conducted to identify 4 randomized clinical trials evaluating the use of intravaginal physical energy in women with non-metastatic breast cancer. Results: A total of 2,588 articles were found, of which only 3 studies met the inclusion criteria. The primary outcome was the assessment of improvement in vaginal atrophy, sexual dysfunction, urinary symptoms, and vaginal health. A total of 185 cancer survivors were included. The studies suggest short-term improvements in GSM symptoms with physical energies; however, no differences were observed between the control groups. The need for additional research is highlighted, as well as the lack of consensus on the best therapy and the necessity for more objective evaluation methods, recognizing the heterogeneity of protocols as a limitation. Conclusion: Due to the heterogeneity of the studies, meta-analysis was not possible, and the efficacy of intravaginal physical therapies for GSM in breast cancer survivors could not be confirmed. However, the studies demonstrated safety. Therefore, more high-quality and homogeneous studies are needed to evaluate efficacy, with similar outcomes and methods.

Rheumatic Fever (RF) is the main cause of acquired heart disease in childhood. Health professionals need to be able and skilled to quickly recognize the signs of the pathology, especially Primary Health Care professionals, considered the gateway to the diagnosis and profile of the disease. However, it is still important in the recognition, diagnosis of the disease, and treatment by health professionals. The development of capabilities for the treatment of RF began to occur, mainly, and the increase in the progression of evolution began to occur. During the pandemic, due to the need for social isolation, the adaptation of health education models became essential. Thus, education to fundamental education will be created through distance education on FR, in the mold of its education through remote project. Therefore, a guide protocol was developed, applied to 36 Medicine and Nursing Students, through a practical simulation carried out in four stages. This education work for remote teaching the transition from the face-to-face model in FR. It showed that the virtual training was complemented to increase the topic addressed on RF, from a theoretical point of view. The region's diagnostic mode and more remote treatment can increase as the regional diagnostic capacity and the increase capacity of the region increase. It was concluded that health education is essential for the development and updating of knowledge about RF; that the remote teaching model is an excellent alternative for the moment of global crisis in which social isolation has become necessary; and that the use of resources contributes to making technological knowledge accessible to a greater number of people, in a clear and universal way.

(Universidade Federal do Rio Grande do Norte, 2017-11) Honorato, Cecília Mirelle Almeida; Machado, Flávia Christiane de Azevedo
Objective: The objective is to analyze the factors that trigger work stress in health professionals who work in emergency medical services. Method: Systematic review of literature using the Lilacs and MedLine databases to search for scientific articles. We included cross-sectional or longitudinal studies in English, Portuguese and Spanish that aimed to evaluate stress levels in health professionals working in emergency medical services. Results: Nine transversal and two longitudinal articles were included. As a result, in emergency medical services, passive work predominates, which is associated with low demand and low control over the work process. In addition, a series of environmental factors (poor working conditions, management activities and conflicting interpersonal relationships) and personal factors (stable union and shorter service time) are associated with work stress. Conclusion: Several factors are associated with work stress; however, several are open to intervention. Thus, there is a need for policies to improve the work environment and the empowerment of the health professional working in emergency medical services.

(Universidade Federal do Rio Grande do Norte, 2018-06-25) Pedrollo, Vinícius Paganini; Souza, Dyego Leandro Bezerra de; Nayara Priscila Dantas de Oliveira
ntroduction: Breast cancer is among the leading causes of death and is the second most common cancer in the world. Socioeconomic factors have been shown to correlate with tumor pathological staging at diagnosis in order to identify populations at risk situations. Objective: To determine the prevalence of advanced staging in the diagnosis of breast cancer and associated factors in women undergoing cancer treatment in the city of Natal, Rio Grande do Norte. Methodology: This is a crosssectional study involving 97 women living in the city of Natal-RN, diagnosed with malignant neoplasm of the breast, who underwent oncologic treatment for at least one year and who are still in clinical follow-up in the Northern League Riograndense against Cancer. The data collection of the study was composed of two phases, one from the access to the patients' medical records and another one from individual interviews. Variables related to socioeconomic characteristics, life habits, health conditions, gynecological and obstetric history, clinical characteristics of the tumor, and therapeutic approach were collected. Bivariate analysis was performed using Pearson's Chi-square test or Fisher's exact test. Results: 11.3% of the women were diagnosed in more advanced stages (CI: 95%), they were older than 55 years (50.5%), white (50.5%), income higher than two monthly minimum wages (61.9%), were married (49.5%), had completed high school education (57.8%), breast selfexamination (66%), mammography after age 40 (73 , 2%) and had access to the health service through the public system (52.6%). Due to the limited sample space, no statistically significant p value was obtained, however the income was the one with the closest value (p = 0.09). Conclusions: A high prevalence of diagnoses in more advanced stages of breast cancer and its relation with socioeconomic factors directly reflects the quality of the cancer screening programs, so that it allows the correct and effective investment in health policies for a greater comprehensiveness of the screening programs and early diagnosis

Inflammatory bowel diseases are chronic inflammatory disorders that affect the gastrointestinal tract. In pediatric patients, about 25%, the disease has a more aggressive nature and a worse prognosis. In addition, they reach them at an important stage of their development, generating significant physical and cognitive deficit. Treatment, when instituted effectively and rapidly, alters the natural history of the disease and prevents its complications. Immunomodulators (azathioprine and methotrexate) are used as first choice in the maintenance of the disease, however, the scientific basis for the management of these drugs comes from studies in adults, and the efficacy and safety of these drugs in pediatric patients remains uncertain. This systematic review aims to systematically evaluate published studies on the efficacy and safety of immunomodulators for the treatment of inflammatory bowel diseases in children. A systematic search was performed in the PUBMED/MEDLINE and BVS/Lilacs databases, with search terms and Boolean operators: ((Inflammatory Bowel Disease) AND (Immunosuppressants OR Azathioprine OR Methotrexate). We included 12 articles with a total population of 1233 patients. Azathioprine and methotrexate have been shown to be safe and effective for the treatment of IBD in children and are recommended for moderate to severe disease, and individual prescription should be assessed.

Theoretical studies hypothesized that the emergence of a columnar functional organization in the primary visual cortex (Area 17 or V1) could be related to the cortical volume, total cortical neurons or cell density in this region. It is very likely that the Dasyprocta species does not present large columns in their visual cortex. Several studies have used the Cavalieri method to estimate cortical volume and the Optical Fractionator to estimate cell density, but there are no published studies which have calculated primary visual cortex’s volume and cell density for Dasyprocta with these methods. Especially, for Dasyprocta leporina there are no data on volume, neuronal density or cellular density of V1 at all. In this study we aimed to estimate the volume and the cell density for Area 17 for the Dasyprocta leporina, an animal whose neuronal properties are also studied with electrophysiological methods in the lab of the Brain Institute. Our methods included area 17 volume and cell density estimation using Nissl stained and DAPI marked 50 μm thick sequential coronal sections, Stereo Zeiss Imager M.2 microscope and Stereo Investigator 10.0 software using Cavalieri stereological method and the Optical Fractionator. Our results are that Dasyprocta leporina’s primary visual cortex has a volume of 545 mm³ ± 42 mm³, 25% ± 10% higher than described in a previous study for the Dasyprocta azarae and total cell density of 82,516 cells/mm², a smaller number than compared to previous data. Because these are different species, there may be also differences in the size of area 17. Furthermore, the previously described animals had smaller visual cortex lengths in the macroscopic view and thinner cortices than ours. Our results indicate that the Dasyprocta leporina can have a considerable V1 volume but a smaller cellular density compared to other species such as cats and monkeys, which present columnar organization. That strengthens the hypothesis that these parameters could be related to the emergence of columnar organization in area 17.

Introduction: Congenital Zika syndrome (CZS) presents a wide spectrum of congenital abnormalities, especially neurological, such as microcephaly. Studies have evaluated congenital heart diseases (CHD) as possible components of this spectrum and have shown an increase in the prevalence of CHD without hemodynamic repercussions in this group. This study aimed to evaluate the frequency of CHD in children with CZS. Methodology: Retrospective cross-sectional study with patients diagnosed with CZS, evaluated between May 2015 and March 2020 at the Multiprofessional Outpatient Clinic for Congenital Infections of a University Reference Service in Natal/RN, Brazil. Results: Fifty-one patients met the criteria for diagnosing CZS and underwent echocardiography; 18 (35.3%) presented altered exams; 10 (19.6%) showed evidence of mild CHD, and no cases presented hemodynamic repercussion or had moderate or severe CHD. No children had serological confirmation for congenital co-infections. Conclusion: Children with CSZ presented a frequency of mild CHD higher than the prevalence of CHD in the Brazilian population; however, no cases were classified as severe CHD or had hemodynamic repercussion. Therefore, it is suggested that cardiological screening for patients with suspected or confirmed intrauterine exposure to Zika virus should follow the same protocol used for the general population.

Cervical Cancer, also called cervical cancer, is a neoplasm that affects the cervix and ranks fourth in the mortality rate from cancer among women around the world, being considered a public health problem. The main causal agent is human papilloma virus (HPV) infection and some risk factors are related to increased contamination by this virus such as age group, exposure to alcohol and/or tobacco, early onset of sex life, multiple partners and low socioeconomic status. In addition, the irregularity in the performance of the preventive examination of oncotic cytology and the consequent late diagnosis have proved relevant for the high rates of morbidity and mortality. The objective of this work was to describe the epidemiological profile of women with Cervical Cancer attended in the Riograndense Northern League Against Cancer. A research on the subject is justified due to the retrospective, quantitative, cross-sectional and descriptive study based on data from the hospital registry of patients followed from January 2012 to December 2016. Regarding the methodology, information was collected from 1234 women aged 18 to 100 years. The highest incidence of neoplasia was between 30-39 years (25.4%). Most were brown (65%) and had incomplete elementary school (45%). Regarding lifestyle, 473 patients (38%) denied a history of alcoholism and 442 (36%) they had no history of smoking, although these data were distorted due to underreporting of 45.5% and 33.2%, respectively. Although the clinical stage of 30% of patients was not reported, the recorded data revealed that stage III was the most expressive (37%). Regarding disease control, after one year of treatment, 659 patients (53%) showed no evidence of the disease. Of the total of 1234 patients 269 (22%) died from cancer during the research period. The intersection between schooling and clinical staging indicated that, in stage III, with a portion of 53% of the cases, 48% had incomplete elementary school, which may have influenced late screening. A relationship was observed between disease control during the first year of treatment and clinical stage, demonstrating that 91% of patients with stage I did not present evidence of disease, while for stage III, this percentage decreased significantly to 45%. It is concluded that the results obtained corroborate the current statistics, which point to an important incidence among women over 30 years of age, as well as the occurrence of death in those who were diagnosed late. Therefore, despite being a disease with a 100% chance of cure, if diagnosed early, there is still a high mortality rate among affected women, which reflects the importance of investing in educational and preventive actions, with the objective of demystify rooted cultures and raise awareness among women about their responsibility for their health. At the same time, the infrastructure of basic health units and professional training should receive special attention so that the practice of the examination is feasible and early prevention or diagnosis is an effective reality.
