In Primary Health Care (PHC), one of the challenges is the fulfillment of its essential attributes, however, there is also concern about the implementation of the guidelines of the National Humanization Policy (PNH), namely: welcoming, ambience, participatory management, defense of users' rights, appreciation of the worker and expanded clinic. Objective: To analyze compliance with PNH guidelines in PHC during the COVID-19 pandemic, from the user's point of view. Methods: Qualitative, exploratory and descriptive study, carried out with users assisted by the Family Health Strategy (ESF) in the municipality of Brejo Santo/CE. The research meets the criteria of resolution n° 466/12 of the National Health Council and was appreciated by the Research Ethics Committee (CEP) of the Faculty of Health Sciences of Trairi (FACISA)/Federal University of Rio Grande do Norte (UFRN ), having obtained a favorable opinion from nº 5.538.037/2022. The collection technique was a semi-structured interview, guided by a script prepared by the research team. The study included users aged 18 years or over, registered in the ESF of the aforementioned municipality for at least two years. Those users who had flu signs or symptoms at the time of the interview were excluded, following the health recommendations adopted during the pandemic. Sampling was carried out for convenience, considering the period established for data collection (between September and December 2022), so that a total of 23 participants was reached. Results: Data were processed using the Iramuteq software and submitted to thematic content analysis. The Descending Hierarchical Classification revealed six classes that originated the thematic categories, namely: "Reception and access in the ESF"; "Ambience and appreciation of the worker as a health service qualification strategy"; and "The importance of co-management and defense of the rights of SUS users". Conclusion: During the COVID-19 pandemic, access to care was maintained, through necessary adaptations due to the health context experienced at the time of collection. Although some users are unaware of the SUS user rights booklet, which can cause weaknesses in popular participation, it was demonstrated, in general, that there was respect for compliance with the PNH guidelines in these services. It was interpreted that welcoming and ambience are guidelines that appear in the speeches through other terms, however, guidelines such as co-management and expanded clinic were not identified. Primary health care has a direct link with public health policies as it is regulated by the National Primary Care Policy. Among the subjects that can be addressed in public policies, the sleep/wake cycle can be highlighted. The damage to health caused by changes in relation to this cycle are increasingly evident. Therefore, the objective of this study was to map evidence on the sleep/wake cycle related to health policies in the context of primary health care. This is a scoping review conducted according to the Joanna Briggs Institute methodology and PRISMA-ScR. The databases consulted were PubMed, LILACS and Scopus, in English and Portuguese publications from the last 5 years. The primary care notebooks on the website of the Secretariat of Primary Health Care of the Brazilian Ministry of Health were also consulted. It was found 990 evidence, 959 articles came from the databases, of which 28 were selected for full reading, and 1 article was found by recommendation, thus 9 articles were included in the study. Among the evidence, 30 were primary care notebooks and of these only 23 were included. The 9 articles analyzed were mostly from North American and European countries and addressed health policies related to: insomnia; sleep apnea; quality of life improvement course; sleep education program; sleep clinic; compliance with national guidelines; life habits and academic performance; and the delay in starting classes. In primary care notebooks, it was observed that 9 guide health professionals to investigate or question patients about sleep during consultations and some inform what professionals should advise about sleep. In addition, sleep or drowsiness was observed in 8 notebooks as an adverse effect of the use of drugs and 10 of them indicated changes in sleep or drowsiness as symptoms resulting from health conditions. It is clear then that the central theme addressed in the articles and notebooks evaluated in this study refer specifically to sleep, treating in a limited way evidence about the sleep/wake cycle and biological rhythmicity in health policies of primary health care. In addition, no health policy related to the sleep/wake cycle was observed in Brazil. In a positive way, it is observed that sleep is a topic addressed in primary care notebooks, but it does not integrate or address the biological rhythm of the most varied body systems, which is fundamental for individual and collective health, as well as the relationship between sleep/wake cycle with health-diseaseillness processes. In addition, physical activity is an important component of quality of life, physical and mental health. However, not everyone has access to environments that facilitate this practice. This is a reality that needs to be better evaluated and that, although it has more defined contours in large urban centers, is relatively little known in small cities. Thus, the objective of the present work is to evaluate public spaces destined to the practice of physical activity in the microregion of Trairi Potiguar. This is a descriptive observational cross-sectional study that evaluated public structures for the practice of physical activity in the 11 counties that make up the micro-region of Trairi, in the state of Rio Grande do Norte. This territory has an estimated population of 125,620 inhabitants for 2021, the predominant biome is the caatinga and the climate is semiarid with long periods of drought. Initially, a survey of the structures destined to the practice of physical activity was carried out with the Municipal Sports Departments or responsible departments and at the time of visits to the municipalities, through information collected with the residents. Courts/gyms, parks for walking, health academies and squares were included. Thus, 79 locations were visited in loco, totaling 98 structures, and aspects related to the repertoire and quality of available equipment, user comfort attributes, aesthetics, safety, services and shading of spaces and equipment were evaluated. Most of the structures were in a good state of conservation, as well as good conditions of cleanliness, aesthetics and safety, benches and good quality lighting, however comfort items such as bathrooms, drinking fountains, shading conditions of the structures, and accessibility require greater attention. The Organizational Readiness for Change (ORC) instrument uses the most relevant domains for the perception and assessment of professionals' propensity to change, containing 12 domains and 65 items. Objective: translate, adapt and validate preliminary data from the instrument Organizational Readiness for Change - Organizational Readiness for Change (POM) for use in Brazil. Materials and methods: this is a pilot study in which 32 health professionals from Tangará-RN participated. The research was approved by the Research Ethics Committee and the participants were contacted personally, via email and/or via WhatsApp, informed about the objectives and signed the informed consent, answering the questionnaire via Google Forms. The instrument is self-administered, answered in 20 minutes and will contribute to the realization of other studies that aim to measure the impact of the proposed change in the studied population. The validation of the instrument's preliminary data was based on the analysis of the psychometric characteristics. For test-retest reliability: ICC; and to calculate the internal consistency of the scale: Cronbach's Alpha, using SPSS for data analysis. Results: 32 professionals (71.11%) accepted and responded to the test, and 29 to the retest, with a loss of 3 professionals. Test: 26 women (81.25%) and 6 men (18.75%), mean age of 32.5 years, mean time since graduation of 6.07 years and mean experience in the area of 5.51 years. Retest: 24 women (82.75%) and 5 men (17.25%), mean age of 32.24 years, mean time since graduation of 5.98 years and mean experience in the area of 5.5 years ; Most nurses (37.50%), dentists (18.75%) and social workers (12.50%) participated in the test; in the retest, most nurses (37.94%), dentists (17.24%) and social workers (13.80%) also participated. Regarding reliability, the POM Assessment showed Cronbach's alpha greater than 0.80 in the test and retest. The test-retest ICC was 0.921, a value considered optimal. Researchers has been dedicating to studying the relationship between FI and Quality of Life (QL), whose results show that the violation of this access to adequate food can cause alarming consequences in people's lives, such as mental health, social disorders, health damage physical and other health problems, affecting subjects in all life cycles. Thus, this study aims to analyze the association between FI and QL of residents of the city of Cuité/PB, from a longitudinal study carried out in 2014 and 2019. The sample of this study consisted of 225 people, residents both in urban and rural areas and the instruments used were the Brazilian Scale of Food Insecurity (BSFI) and the WHOQOL-bref. An exploratory analysis was performed based on central measures and data dispersion, of the bivariate type, considering sociodemographic characteristics and mean QL scores, and of the multivariate type from linear longitudinal regression models for fixed effects, and it was from this that predicted values of quality of life outcomes were generated according to family AI levels. It was noticed that, in both years studied, the QL domain with the lowest score was environment and the highest score was social relations. It is noteworthy that there was a significant reduction in the mean score in the Physical domain between baseline and follow-up (-2.9) and stagnation in the other mean scores in the social relations, psychological and environment domains. Based on the statistical analysis for panel data, there was an inverse association between the scores of the Psychological and Environment domains and Severe AI, that is, in situations of severe AI, there was a reduction in the QL scores of these domains by - 11.222 points and -7.987 points, respectively. In this sense, it was noted that in the Psychological domain, the predicted mean scores are similar for the categories of Food Security (FS), Mild and Moderate Food Insecurity, significantly reducing the situation of severe FI. In the case of the environment domain, the predicted mean scores decrease in the moderate and severe FI category, when compared to the scores in the FS and mild FI. Finally, in the Physical and Social Relations domains, this effect of FI in reducing the score was not observed. The results presented in this cohort confirm the hypothesis that the FI phenomenon negatively impacts QL outcomes over time.Master Thesis Avaliação em saúde aplicada aos Centros Especializados em Reabilitação: proposição e validação por consenso de um modelo lógico(Universidade Federal do Rio Grande do Norte, 2020-08-11) Valentim, Regis de Souza; Dantas, Diego de Sousa; ; ; Coura, Alexsandro Silva; ; Machado, Flávia Christiane de Azevedo;Introduction: Specialized Rehabilitation Centers (CER) are health services, qualified by the Ministry of Health, and responsible for specialized care for people with disabilities. Objective: Analyze the work process and the organizational issues of CER, propose and validate by consensus a logical model for evaluating the service. Method: Methodologic study developed in three stages: 1) Documentary study of laws and ordinances related to the health service and Care Network for People with Desabilities (RCPD); 2) Conducting focus groups, with a census study of CER in the state of Rio Grande do Norte, to assess and understand the organization of the work process in the daily service; 3) Systematization of the collected information, proposition and validation of the logical evaluation model. This study included managers and professionals from all CER that make up the state’s RCPD. Results: The model encompassed five central categories of the work process and organizational issues: demands, resources (inputs, financial and workforce), processes, products and results, as well as mission, values and external factors. Conclusion: The proposed logical model was valid for graphic representation of the work process and organizational issues of CER. It became evident that the functioning of the services is aligned with the norms, however organizational gaps need to be addressed in order to improve the resoluteness of the service's actions and the articulation with other points in the network.Master Thesis Prejuízos na qualidade de vida e funcionalidade de adultos cronicamente afetados pela febre Chikungunya(2019-07-15) Dutra, Jéssica Isabelle dos Santos; Medeiros, Anna Cecilia Queiroz de; Souza, Marcelo Cardoso De; ; ; ; Knackfuss, Maria Irany; ; Câmara, Saionara Maria Aires da;Introduction: In 2016, Rio Grande do Norte recorded a major epidemic of chikungunya fever (FChik), with an incidence rate almost six times higher than national and 37 confirmed deaths the disease. In addition, One of the biggest challenges in the treatment of FChic is the possibility of symptom chronicity, since all the characteristics of the clinical phase are not well understood. Objective: To analyze the effect of chronicity of chikungunya fever on quality of life and functionality of those affected. Materials and methods: This is a descriptive and comparative cross-sectional study, carried out in two stages. The first was a descriptive study of 103 adults confirmed for FChik in Natal-RN, traced at least one year after getting sick. Clinical, sociodemographic and telephone data were collected through the Individual Notification Form (FNI) and the telephone survey was conducted to collect aspects of the chronic phase. There was success in the rescue of 33 patients who were interviewed, and of these 25 chronified. The second stage was a comparative study, conducted with 2 groups: chikungunya group (GChik), composed of 25 chronic patients of FChik (from stage 1); and the healthy group (GS), consisting of 25 healthy individuals, recruited for convenience and matched for gender and age. Both groups answered the Stanford HAQ 20-Item Disability Scale (HAQ) and SF-12v2 Short Form Health Survey (SF-12) questionnaires. Results: In the first stage, the most prominent joint symptoms in the acute phase were: arthralgia (77.66%), back pain (67.96%) and arthritis (63.10%); Already in the chronic phase were joint pain (92%), periarticular edema (56%) and limitation of movement (40%). Regarding work, 65.21% (n = 15) reported damages and 82.60% (n = 19) needed removal time off work, with absenteeism, mostly, between 7 and 30 days. In the second stage, significant differences were found between GChik and GS, both for functional capacity and quality of life (p <0.05). The most compromised aspects were HAQ Walking category, the Body Pain domain and SF-12 mental component summaries. Still, GChik presented a greater chance for the development of depression. Conclusions: FChik compromises the functionality and quality of life of those affected, with direct impairment in the autonomy to perform daily activities and risk of developing other morbidities, especially related to mental health.Master Thesis Avaliação da saúde bucal autopercebida e clínica de idosos institucionalizados e residentes nas comunidades urbanas e rurais do interior da Paraíba(2018-08-02) Costa, Moan Jéfter Fernandes; Souza, Marcelo Cardoso de; ; ; Guedes, Dimitri Taurino; ; Guênes, Gymenna Maria Tenório;Objective: To evaluate the self-perception of oral health in the physical and psychosocial dimensions, besides the orofacial pain of institutionalized elderly people, living in the urban area and rural area in the interior of Paraíba. Method: Observational, cross-sectional study, quantitative approach, population base and non-probabilistic sampling for convenience among 81 elderly: 27 residents in a long-stay institution and the other males by age and gender between urban (n = 27) and rural (n = 27). Two validated questionnaires were used, one of them the GOHAI (Oral Health Assessment Index) for the evaluation of self-perception of oral health in quality of life and the other, QST / DTM (Questionnaire for Screening of Patients with Temporomandibular Disorders) for influences of the orofacial pains and the indices of biofilm in tooth proposed by Silness and Loe and in prosthetics by Ambjornsen. Results: The data were studied through a descriptive and inferential analysis, with a predominance of females (74.1%), 77.8% of whom were born in the city of Cuité-PB and a higher frequency (32.1%) of age range of 60-65 years. The frequency of GOHAI related to high perception obtained a higher frequency between the urban area (77.8%) and lower frequency (66.7) in the rural area, with a p-value of 0.004, showing a statistical difference among the dwelling places. In the QST / DTM, all frequencies indicated the majority of individuals as non-carriers, but with a statistical relation related to gender (p = 0.007) and income (p = 0.002). The highest frequencies for the biofilm analysis were related to the presence of this in teeth or prosthesis, more frequently linked to the inhabitants of the rural area, 84.6%. Conclusion: There was a difference between the samples related to self-perception and the place of residence, showing the elderly with excellent oral health, not consistent with the clinical condition found, showing the secondarily of oral health problems.Master Thesis Crescimento e desenvolvimento de crianças com microcefalia associado a Síndrome Congênita do Zika vírus no Brasil(2018-06-18) França, Thaís Lorena Barbosa de; Sousa, Klayton Galante; ; ; Hull, Egmar Longo; ; Medeiros, Wilton Rodrigues; ; Cunha, Andrea Baraldi;Introduction: The outbreak of Zika virus in Latin America in the period 2015-2016 has caused a sudden increase in the number of severe manifestations and reports of congenital changes in newborns in Brazil. Objective: This is the first study that evaluated and compared the growth and cognitive and motor development of children with microcephaly due to Congenital Zika Virus Syndrome (CZS) in relation to typical children. Methods: An observational, analytical, cross-sectional study with eight children with CZS and 16 typical children, with a mean age of 20.5 months (± 2.1) in a region of northeastern Brazil. Results: Considering the mean, children with CZS presented extremely low performance in the motor domain and in the cognitive development domain, whereas typical children presented average performance in the cognitive and motor development domains. Conclusion: Children with CZS presented a mean growth rate (head circumference and weight) lower than typical children. Therefore, children with CZS are at risk for growth retardation and development compared to typical children.
