These individuals must follow therapeutic pathways with defined flows between health devices, optimizing their journey and integrating care. Objective: To provide a report on the experience of implementing the Overweight and Obesity Care Line in the Nutrition Programs core of the Municipal Health Secretariat of Currais Novos during the rotation of secondary care services between March and September 2024. Methodology: The report is organized into topics that follow the chronological order of activities carried out in the nutrition core, such as the organization of the Care Line in the municipality; the structuring of group work in primary care; the engagement of network nutritionists; and data collection within the territory concerning individuals with overweight and obesity. Results: It was identified that the Overweight and Obesity Care Line in the municipality of Currais Novos has weaknesses at several points, such as ineffective communication between network services, the cultural predominance of bariatric surgery in the municipality, difficulties in implementing group care in Primary Health Units, and low community participation in collective actions and care groups.
