    Natural lipid-based nanoparticles with anti-inflammatory properties: quality control, safety and efficacy

    Inflammation is an evolutionarily conserved biological response that can be persistent or dysregulated. It also contributes to the pathophysiology of several chronic diseases. The conventional pharmacological therapy is based on the use of non-steroidal anti-inflammatory drugs (NSAIDs) and glucocorticoids, causing gastrointestinal, renal, cardiovascular, hepatic and metabolic side effects.Therefore, the search for safer and more effective alternative treatments has become a relevant research focus. Essential oils (EO) have emerged as promising multi-target anti-inflammatory agents, capable of modulating inflammatory mediators such as: TNF-α, IL-1β, IL-6, NF-κB, COX-2 and iNOS. However, its pharmaceutical application is limited by intrinsic physicochemical instability, volatility, hydrophobicity and low bioavailability. This review analyzed studies on essential oil-loaded lipid-based nanoparticles with anti-inflammatory activity. It emphasized data regarding on quality control parameters, physicochemical stability and in vitro and in vivo safety and efficacy assays. A systematized search was conducted through PubMed/MEDLINE, PubMed Central, ScienceDirect, SciELO, Virtual Health Library and Google Scholar databases, following the recommendation of PRISMA 2020 statement. The main lipid nanocarriers described for this purpose were liposomes (LUV), solid lipid nanoparticles (SLN), nanostructured lipid carriers (NLC) and nanoemulsions (NE). It was observed that across the acute, subacute and chronic experimental models, the nanoencapsulation consistently enhanced the anti-inflammatory effect of EO. Several formulations have reached efficacy results comparable to hydrocortisone, diclofenac or NSAIDs. Such systems seem to act on signaling pathways as NF-κB, MAPK and Keap1/Nrf2/HO-1. The consolidation of these systems as anti-inflammatory nanodrugs still have to overcome some issues. The absence of harmonized phytochemical standardization, persistent gaps in the nano-specific regulatory framework and the scarcity of clinical trials can delay the approval of such nanosystems. Lipid nanocarriers represent robust and viable platforms integrating the bioactive potential of EO into nanoparticles, resulting in stable, safe and efficient anti-inflammatory candidates for both human and veterinary clinical uses

    Commiphora leptophloeos (Mart.) J.B. Gillett (Burseraceae) is a species native to the Caatinga biome, widely used in folk medicine to treat inflammatory, infectious, and respiratory diseases. Phytochemical studies have demonstrated the presence of tannins in the species' bark, notably the B2-type dimeric procyanidin (PDB2), which is the major constituent and a chemical marker of the bark extract. This study aimed to evaluate the influence of seasonality on the levels of total phenolic compounds and PDB2 in C. leptophloeos bark extracts collected between August 2023 and August 2025. Total phenolic compounds were determined using the Folin-Ciocalteu method, and PDB2 was quantified via High-Performance Liquid Chromatography coupled with a Diode Array Detector (HPLC-DAD). Statistical analyses compared dry and rainy seasons, and monthly variation was assessed using August 2023 as a reference. Regarding total phenolic compounds, the second dry season showed the highest mean content (332.14 mg GAE/g of extract), differing statistically from the first dry season and the first rainy season, while monthly analysis revealed higher levels during dry periods. PDB2 also showed the highest mean content during the second dry season (12.05 mg/g of extract); however, only the first dry season differed statistically from the others, and monthly analysis demonstrated a wider range of variation, with no consistent trend regarding the meteorological parameters evaluated. Despite the observed quantitative variations, the extracts maintained the species' characteristic chromatographic profile and consistent production of the evaluated metabolites, a favorable result for quality control. Preliminary results suggest that the dry season is the most favorable period for collecting the bark, due to the higher levels of total phenolic compounds observed.

    The human papillomavirus (HPV) is a highly prevalent sexually transmitted infection, and the persistence of high-risk genotypes is associated with the development of cervical cancer. Current management relies on invasive procedures and topical therapies, which exhibit limited efficacy and significant adverse effects. Rutin, a flavonoid with antioxidant and anti-inflammatory properties, emerges as a promising alternative. This study aimed to develop and characterize vaginal films based on Hydroxypropylmethylcellulose (HPMC) and tilapia gelatin (Oreochromis niloticus) containing rutin. The films were prepared using the solvent casting technique and characterized regarding visual appearance, average weight, pH, and disintegration time. The results demonstrated that the incorporation of rutin imparted a light-yellow color to the films and did not significantly affect the pH of the film-forming solutions, which remained within the range of 4 to 5, compatible with the vaginal environment. After film disintegration in an aqueous medium, the resulting dispersions exhibited pH values of 6.38 and 6.60. Furthermore, rutin increased the disintegration time by up to 1.4-fold, from 62.76 ± 0.08 minutes to 88.04 ± 0.07 minutes. The low variability among the samples indicated good reproducibility in the manufacturing process of the proposed devices. In conclusion, the rutin-loaded vaginal films are technologically feasible and exhibit adequate preliminary physicochemical properties, representing a potential alternative for complementary physicochemical studies and in vitro cellular assays aimed at the management of HPV-associated lesions

    Objective: To characterize the requests handled by a Drug Information Service (DIS) and analyze their impact on clinical practice in a teaching maternity hospital. Method: Observational study based on the analysis of records of requests handled between May 2024 and July 2025. Variables analyzed included requester type, originating department, priority level, request method, response time, medication classification, patient involvement, and impact on clinical practice, using descriptive statistics. Results: A total of 53 information requests were analyzed, with pharmacists being the main requesters (47.17%). The Clinical Pharmacy Unit was the sector that most frequently requested the service (43.40%), followed by the maternal and neonatal intensive care units. E-mail was the main method used both for receiving (56.6%) and responding to requests (66%). Urgent requests accounted for 15.10%, all of which were answered within 2 hours. Among normal requests (84.9%), 40% were answered after 2 hours, while the others required a longer response time. Antimicrobials were the most frequently involved drugs (30.19%). Drug package inserts available in the Brazilian Health Regulatory Agency (ANVISA) database were the most consulted sources (21.90%), being directly related to the most recurrent topic, drug stability (33.96%). It was observed that 32.10% of the requests were related to hospitalized patients. Regarding support for clinical practice, assistance to clinical pharmacy activities and pharmaceutical dispensing stood out (52.83%), in addition to support for medication administration (28.30%) and prescription support (9.43%). Conclusion: The analysis of the requests made it possible to characterize the service profile and highlight its potential support for clinical practice and decision-making, reinforcing the essential role of the Medication Information Service (SIM) in promoting the rational use of medicines, patient safety, and support for therapeutic decisions in the hospital setting.

    Cannabidiol-loaded microemulsion: first steps toward biosafety evaluation

    Cannabidiol (CBD) is a highly lipophilic molecule with promising therapeutic potential; however, its clinical application is limited by its poor aqueous solubility and low bioavailability. Lipid-based nanostructured systems, mainly microemulsions (ME), have emerged as effective strategies to overcome these limitations by enhancing drug solubilization, stability, and absorption. In addition, their nanoscale droplet size enables the development of formulations suitable for alternative administration routes, including parenteral delivery. In this context, this study aimed to produce and characterize a CBD-loaded microemulsion (CBD-ME) and evaluate its biosafety by hemolysis assay. The microemulsion was produced by a high-energy method, and CBD was added via direct entrapment. The system was characterized by droplet size, polydispersity index (PdI), pH, conductivity, entrapment efficiency and hemolytic potential. The results showed that ME and CBD-ME were optically clear, homogeneous and translucent system. Following 30 days of stability evaluation at room temperature, the ME system showed (i) average droplet size of 23.79 ± 0.99 nm; (ii) PdI of 0.233 ± 0.008; (iii) conductivity of 129.95 ± 1.08; and (iv) pH of 5.92 ± 0.07. Likewise, the CBD-ME presented: (i) average droplet size of 28.31 ± 0.29 nm; (ii) PdI of 0.291 ± 0.015; (iii) conductivity of 121.10 ± 0.97; (iv) pH of 5.83 ± 0.11; and (v) entrapment efficiency of 96.8 ± 0.7%. The formulation exhibited low hemolytic activity (< 1%), suggesting favorable biocompatibility. These data indicate the successful production of CBD-ME with characteristic size and physicochemical properties suggestive of a microemulsion. CBD entrapment did not affect the physicochemical properties of the system. In addition, the absence of hemolytic activity supports its biocompatibility and safety, while the developed microemulsion represents a promising strategy to improve CBD delivery and overcome biopharmaceutical limitations

    Introduction: The management of chronic liver disease (CLD) is commonly associated with polypharmacy, which is the primary factor related to the occurrence of drug-drug interactions. Monitoring interactions in these patients must consider the liver's reduced capacity to metabolize and clear drugs. Objective: To characterize the occurrence and profile of drug interactions in patients with CLD. Methods: This prospective observational cohort study, conducted with 62 patients hospitalized between August 2023 and January 2025, evaluated the occurrence of drug interactions in adults with moderate or severe hepatic impairment (Child-Pugh B or C). Data collection via electronic medical records encompassed diagnoses, laboratory tests, clinical complications, and medical prescriptions. Moderate and severe drug interactions were identified from admission to discharge using the Drugs.com platform. Results: Alcoholic liver disease was the main etiology (61%), with a predominance of Child-Pugh B classification (62.5%) and a mean MELD score of 17.6. The most frequent complications were ascites (74.4%), jaundice (45.1%), and hepatic encephalopathy (45.1%). Mean levels of albumin (2.8 g/dL) and creatinine (1.6 mg/dL) reinforce the impairment of hepatorenal function. A total of 2,432 interactions were identified, with 100% of patients experiencing at least one moderate or severe event. The pair of tramadol and ondansetron accounted for 9.1% of severe interactions, while furosemide and tramadol represented 4.3% of moderate ones. Conclusion: Patients with liver disease face a high risk of severe drug interactions due to inevitable polypharmacy. The potential overlap of these events with the pathophysiology of liver disease complicates clinical management and the identification of the etiology of clinical manifestations, increasing the likelihood of unfavorable clinical outcomes.

    Pomada fitoterápico a partir do extrato da casca da imburana (Commiphora leptophloeos) para aplicação tópica pré-clínica: desenvolvimento e caracterização físico-química

    Commiphora leptophloeos (imburana) is a species with recognized healing potential. This study aimed to develop and physicochemically characterize phytotherapeutic ointments from the dried bark extract of C. leptophloeos for future in vivo application. The lyophilized extract was incorporated into a polyethylene glycol base (PEG 4000 at 22%) at three concentrations: 2.5% (F1), 5% (F2), and 10% (F3). The formulations were evaluated for macroscopic aspects, pH, and quantification of the marker (procyanidin dimeric type B - PDB) by UFLC-DAD (CLUE-DAD). In the results, formulations F2 and F3 were shown to be homogeneous, reproducible, and had a pH (4.41 and 4.39, respectively) compatible with the skin. F1 demonstrated physical instability and lack of reproducibility, being considered unviable. UFLC-DAD analysis confirmed that the PDB concentration was directly proportional to the incorporated bioactive content, attesting to the marker's stability. It is concluded that formulations F2 and F3 are stable, possess a biocompatible pH, and proven incorporation of the bioactive, showing promise for in vivo studies.

    The improper disposal of medications poses a significant risk to the environment and public health, compromising ecosystem balance. Brazilian Legislation No. 10.388/2020 establishes the reverse logistics system for household medicines, aiming to ensure their environmentally sound collection and final disposal. This study evaluated the effectiveness of reverse logistics for medicines in Brazil by comparing the performance of the national system LogMed with the local initiative Descarte Consciente of the Federal University of Rio Grande do Norte (UFRN), between 2022 and 2023. The research adopted a descriptive and comparative approach, based on secondary data from LogMed and primary data from the UFRN project. Results indicated that, although LogMed expanded its national coverage, its performance in Rio Grande do Norte declined. Conversely, Descarte Consciente achieved an 84.5% increase in collection efficiency per drop-off point and a 15% higher performance than the national average. It is concluded that the effectiveness of reverse logistics depends not only on infrastructure but also on factors such as environmental education, community engagement, and accessibility, highlighting the importance of integrating public policies with local actions to enhance pharmaceutical waste management.
