Artigo 30-Month randomised clinical trial to evaluate the clinical performance of a nanofill and a nanohybrid composite(2011) Andrade, Ana Karina Maciel de; Duarte, Rosângela Marques; Silva, Fábia Danielle Sales Cunha Medeiros e; Batista, André Ulisses Dantas; Lima, Kenio Costa de; Pontual, Maria Luíza dos Anjos; Montes, Marcos Antonio Japiassú ResendeObjective To evaluate the clinical performance of a nanofill and a nanohybrid composite in restorations in occlusal cavities of posterior teeth in a randomised trial over 30 months. Methods Forty-one adolescents participated in the study. The teeth were restored with a nanofill (Filtek Z350, 3M ESPE), a nanohybrid (Esthet-X, Dentsply); Filtek Z250 (3M ESPE) was used as a control. After 30 months, the restorations were evaluated in accordance with the US Public Health Service (USPHS) modified criteria. The McNemar and Friedman tests were used for statistical analysis, at a level of significance of 5%. Results There were significant differences in the roughness of Filtek Z250 (p = 0.008) and Filtek Z350 (p < 0.001) when the four time periods (baseline, 6 months, 12 months and 30 months) were compared. There were significant differences in the marginal adaptation of Filtek Z250 (p = 0.001), Filtek Z350 (p < 0.001) and Esthet-X (p = 0.011). Except for one of each composite restoration, all the modifications ranged from Alpha to Bravo. There were significant differences in the surface roughness (p = 0.005) when the three composites were compared after 30 months. Conclusions The materials investigated showed acceptable clinical performance after 30 months. Long-term re-evaluations are necessary for a more detailed analysis of these composites (CEP: #1252).Artigo A 38-year review of oral schwannomas and neurofibromas in a Brazilian population: clinical, histopathological and immunohistochemical study(2011) Nascimento, George João Ferreira do; Rocha, Danielle de Albuquerque Pires; Galvão, Hebel Cavalcanti; Costa, Antonio de Lisboa Lopes; Souza, Lelia Batista deThe benign peripheral nerve sheath tumours are rare lesions mainly represented by schwannoma and neurofibroma. The present work evaluated the clinical and histopathological features of schwannomas and neurofibromas of the oral cavity diagnosed in a Brazilian population. Among 9.000 cases of oral lesions archived from 1970 to 2008, four schwannomas and 12 neurofibromas were identified, microscopically revised and immunohistochemically certified through a panel including monoclonal antibodies (anti-S100, vimentin, HHF-35 and desmin). From biopsy and histological sections records, clinical and histopathological data were retrieved, reviewed and statistically analysed. Predominantly, schwannomas affected non-white males (3:1), with an age and size averages of 34.7 years and 2.8 cm, respectively. Neurofibromas preferentially occurred on the gingival/alveolar ridge of white females (5:1), with 35.7-year mean age, peak of incidence between 3rd and 5th decade, and size average of 1.7 cm. (12 cases, 75%). The studied tumours exhibited more frequently as a painless, sessile and slow growth very similar to other oral lesions, but their microscopic features differed significantly. Schwannomas and neurofibromas are extremely uncommon in the oral cavity, exhibiting clinical features very similar but specific and peculiar microscopic findings that are useful in the establishment of the diagnosis, which in some particular cases must be confirmed by immunohistochemistry. Literature is scarce on the evaluation of disability in Brazilian institutionalized older people. Methods: A cross-sectional study was conducted in 10 nursing homes in the city of Natal (Northeast Brazil). Sociodemographic and health data were collected and activity limitations were assessed by the Katz Index. Results and Discussion: The prevalence of activity limitations was 72.9% (95% confi dence interval: 67.8-77.5) and the most affected activity was “bathing” (71.6%), followed by “dressing” (65.4%) and “toileting” (62.0%). The fi nal model revealed associations with private (for-profi t) institutions (odds ratio [OR] = 3.33; P < .001), age ≥ 83 years (OR = 2.34; P = .003), institutionalization due to lack of caregiver (OR = 1.80: P = .033), and presence of osteoporosis (OR = 2.94; P = .045), adjusted by sex. Conclusions: Activity limitations affected almost 73% of institutionalized older people in this sample and were associated with private nursing homes, age, osteoporosis, and institutionalization due to lack of caregiver. Activity limitations exact high socioeconomic impacts and affect the quality of life of older people. The results obtained herein emphasized the importance of planning strategies for their prevention and treatment, directed to reduce the prevalence of this health issue. The chronic hyperglycemia of diabetes mellitus induces events that promote structural changes in various tissues and are associated with problems in wound healing. This infection remains largely unknown to most clinicians because of its different presentations, and palatal involvement is extremely rare. This report describes the case of a 46-year-old woman who was diagnosed with actinomycosis involving the hard palate. The main clinical, histopathologic, and therapeutic characteristics and differential diagnosis of actinomycosis are reviewed. To date, 3 cases of actinomycosis involving the hard palate have been reported. Artigo Adenomatoid odontogenic tumor associated with dentigerous cyst - unusual case report(2007) Nonaka, Cassiano Francisco Weege; Souza, Lélia Batista de; Quinderé, Lêda BezerraThe adenomatoid odontogenic tumor is a relatively uncommon lesion which mainly affects females in their second decade of life, exhibiting predilection for the anterior region of the maxilla. The lesion is usually associated with the crown of an enclosed tooth, most commonly the maxillary canine. In this paper we present a case of adenomatoid odontogenic tumor associated with a dentigerous cyst affecting the left maxillary region in a 13-year-old female. The authors also discuss clinical, radiographic, histopathologic and therapeutic features of the case.Artigo Adenomatoid odontogenic tumor: retrospective study of 15 cases with emphasis on histopathologic features(2012) Matos, Felipe Rodrigues de; Nonaka, Cassiano Francisco Weege; Pinto, Leão Pereira; Souza, Lelia Batista de; Freitas, Roseana de AlmeidaAdenomatoid odontogenic tumor (AOT) is an uncommon benign epithelial lesion of odontogenic origin and, thus far, only few studies regarding the frequency of its many histopathologic features have been published in the literature. Thus, the aim of this study was to perform a retrospective analysis in a case series of AOT, with emphasis on the histopathological features. Fifteen cases of AOT were studied considering their clinical, radiographic and histopathologic aspects. Twelve cases affected females and the mean age was 16.2 years. The anterior maxilla was the most common site (66.6 %) and radiographically most cases showed a unilocular radiolucency with well-defined borders (57.1 %). Histologically, most cases exhibited predominantly a solid growth pattern (46.7 %) or a similar proportion of solid and cribriform patterns (46.7 %). Eosinophilic amorphous material ("tumor droplets") was found in all cases (100 %). Most tumors showed duct-like spaces (93.3 %) and convoluted structures (60.0 %) whereas a minor proportion of cases presented calcifying epithelial odontogenic tumor (CEOT)-like areas (26.7 %). Variable amounts of calcified material were found in most AOTs (80.0 %) whereas osteodentin and perivascular hyalinization were seen only rarely (6.7 % each one). Five (33.3 %) cases had areas mimicking a dentigerous cyst and most of these were diagnosed in females (80.0 %). Regarding the histopathologic features, our results suggest that AOTs usually show predominance of solid pattern or a similar proportion of solid and cribriform patterns while osteodentin and perivascular hyalinization are rarely seen in these tumors. In addition, areas mimicking a dentigerous cyst and CEOT-like areas are relatively infrequent findings in AOTs.Artigo Advanced caries microbiota in teeth with irreversible pulpitis(2015) Rôças, Isabela N.; Lima, Kenio Costa de; Assunção, Isauremi Vieira de; Gomes, Patrícia N.; Bracks, Igor V.; Siqueira, José F.INTRODUCTION: Bacterial taxa in the forefront of caries biofilms are candidate pathogens for irreversible pulpitis and are possibly the first ones to invade the pulp and initiate endodontic infection. This study examined the microbiota of the most advanced layers of dentinal caries in teeth with irreversible pulpitis. METHODS: DNA extracted from samples taken from deep dentinal caries associated with pulp exposures was analyzed for the presence and relative levels of 33 oral bacterial taxa by using reverse-capture checkerboard hybridization assay. Quantification of total bacteria, streptococci, and lactobacilli was also performed by using real-time quantitative polymerase chain reaction. Associations between the target bacterial taxa and clinical signs/symptoms were also evaluated. RESULTS: The most frequently detected taxa in the checkerboard assay were Atopobium genomospecies C1 (53%), Pseudoramibacter alactolyticus (37%), Streptococcus species (33%), Streptococcus mutans (33%), Parvimonas micra (13%), Fusobacterium nucleatum (13%), and Veillonella species (13%). Streptococcus species, Dialister invisus, and P. micra were significantly associated with throbbing pain, S. mutans with pain to percussion, and Lactobacillus with continuous pain (P < .05). Quantitative polymerase chain reaction revealed a mean total bacterial load of 1 × 10(8) (range, 2.05 × 10(5) to 4.5 × 10(8)) cell equivalents per milligram (wet weight) of dentin. Streptococci and lactobacilli were very prevalent but comprised only 0.09% and 2% of the whole bacterial population, respectively. CONCLUSIONS: Several bacterial taxa were found in advanced caries lesions in teeth with exposed pulps, and some of them were significantly associated with symptoms. A role for these taxa in the etiology of irreversible pulpitis is suspected.Artigo Aggressive olfactory neuroblastoma invading the oral cavity: report of a rare case and review of the literature(ELSEVIER, 2012) Sarmento, Dmitry José de Santana; Henriques, Águida Cristina Gomes; Carvalho, Cyntia Helena Pereira de; Morais, Maria de Lourdes Silva Arruda de; Costa, Antonio de Lisboa Lopes; Silveira, Ericka Janine Dantas daEsthesioneuroblastoma, or olfactory neuroblastoma (ONB), is a rare malignant tumor of the olfactory neuroepithelium, which is normally confined to the superior one third of the nasal septum, cribriform plate, and superior turbinates but may extend to the base of the skull and intracranial space. ONB accounts for only 3% of all intranasal tumors, and its etiology remains unclear. The tumor can affect both children and adults, with a bimodal peak incidence between 11 and 20 years and between 51 and 60 years.1 Controversy exists regarding the origin, diagnosis, and management of ONB, mainly because of its rarity and the fact that most institutions treat only a small number of patients with this diagnosis.2; 3 ; 4 Many cases go unrecognized because of difficulties in the diagnosis of this tumor as a result of nonspecific initial symptoms (nasal obstruction, recurrent epistaxis). There are rare reports of ONB arising in the nasopharynx, ethmoid sinus, and maxillary sinus. We report a rare case of a very large ONB in a pediatric patient. The clinical appearance of the tumor and diagnostic procedure are discussed, and cases published in the literature are reviewed.Artigo Alterations in the immunoexpression of claudin-1 between different grades of oral epithelial dysplasias(2010) Carvalho, Marianne de Vasconcelos; Pereira, Joabe dos Santos; Costa, Antonio de Lisboa Lopes; Souza, Lelia Batista de; Freitas, Roseana de Almeida; Miguel, Marcia Cristina da CostaClaudins are transmembrane proteins that play a role in cell proliferation and adhesion and tumourigenesis. This study evaluated the immunoexpression of claudin-1 in the oral epithelial dysplasia (OED) (19 mild, 26 moderate, 3 severe). Diffuse staining predominated in mild (89.4%) and moderate (80.8%) OEDs. Immunoexpression in the middle and upper third was observed in all mild cases, whereas in moderate/severe dysplasias staining was observed in the upper third in 41.4% of cases, in the upper and middle third in 41.4%, and in the upper, middle and lower third in 17.2% (P<0.05). All mild OEDs and 73.1% of moderate cases presented only membrane staining, whereas membrane/cytoplasmic staining was observed in severe cases. Staining intensity was weak in 60% of parakeratinized OEDs and moderate/strong in orthokeratinized OEDs (60.8%) (P>0.05). The differences in the immunoexpression of claudin-1 between different grades of OEDs suggest the involvement of this protein in the progression of dysplasias.Artigo Ameloblastic fibro-odontoma: Report of a case presenting an unusual clinical course(ELSEVIER, 2006) Soares, Rosilene Calazans; Godoy, Gustavo Pina; Carlos Neto, João; Souza, Lélia Batista de; Pinto, Leão PereiraAmeloblastic fibro-odontoma has been defined by the World Health Organization (WHO) as ‘‘a lesion resembling ameloblastic fibroma’’ which also shows inductive alterations that lead to the formation of enamel and dentin. It is a rare lesion and is classified among odontogenic lesions as an epithelium tumor containing ectomesenchyme with or without the formation of dental hard tissue. We report here the case of a unusual ameloblastic fibro-odontoma seen in a 4-year-old boy and discuss the clinical, radiographic, histopathologic and therapeutic aspects of this tumor.Artigo Ameloblastoma and adenomatoid odontogenic tumor: the role of alpha2beta1, alpha3beta1, and alpha5beta1 integrins in local invasiveness and architectural characteristics(2007) Andrade, Emanuel Sávio de Souza; Miguel, Marcia Cristina da Costa; Pinto, Leão Pereira; Souza, Lélia Batista deAmeloblastoma is an odontogenic neoplasm characterized by local invasiveness and a tendency toward recurrence, whereas adenomatoid odontogenic tumor (AOT) is an indolent neoplasm. The objective of the present study was to immunohistochemically analyze the role of a2b1, a3b1, and a5b1 integrins in the cellular events and cell-matrix interactions that occur in these tumors and their consequent repercussions on the architectural arrangement and biologic behavior of these lesions. Paraffin-embedded specimens from 30 ameloblastomas (20 solid and 10 unicystic tumors) and 12 AOTs were submitted to immunohistochemistry using the catalyzed signal amplification system. A difference in the pattern of integrin expression was observed between the various histologic types of ameloblastoma. No significant difference in labeling intensity was observed between unicystic and solid ameloblastomas, but comparison between ameloblastomas and AOT showed a significantly stronger expression of a5b1 integrin in the former ( P b .05). Our findings suggest an important role of the integrins studied in the architectural characteristics of ameloblastomas and AOTs and a possible participation of a5b1 integrin in the mechanism of local invasion of ameloblastomasArtigo Ameloblastomas sólidos: estudo retrospectivo clínico e histopatológico em 54 casos(2010) Fulco, Giovana Medeiros; Nonaka, Cassiano Francisco Weege; Souza, Lélia Batista de; Miguel, Márcia Cristina da Costa; Pinto, Leão PereiraRecentemente, a Organização Mundial da Saúde (OMS) excluiu o padrão desmoplásico do espectro histopatológico do ameloblastomas sólidos e o enquadrou como uma variante distinta, designada ameloblastoma desmoplásico. Cinquenta e três casos (98,1%) afetaram a mandíbula. Materials and Methods: Sixty-two cases of squamous cell carcinoma of the tongue were selected and examined according to the system established by Brandwein-Gensler et al (Am J Surg Pathol 29:167, 2005) and included the pattern of invasion (most to least favorable), lymphocytic infiltration, perineural invasion, risk score, keratinization, eosinophilia, perivascular invasion, and tumor thickness. Results: The least favorable pattern had no association with nodal metastasis (P .05). The scarcity or density of the lymphocytic infiltration, perineural invasion, and a risk score 3 were associated with nodal metastasis (P .05). Keratinization, eosinophilia, perivascular invasion, and tumor thickness had no association with nodal metastasis (P .05). A significant positive correlation was found between the pattern of invasion and perineural invasion and between the pattern of invasion and tumor thickness (P .05). Conclusions: The scarcity or density of the lymphocytic infiltration, perineural invasion, and histopathologic risk score may be helpful as parameters of histologic malignancy for the evaluation of metastatic and nonmetastatic squamous cell carcinoma of the tongue.Artigo Analysis of local immunity in squamous cell carcinoma of the tongue and lower lip(2010) Silveira, Ericka Janine Dantas da; Miguel, Marcia Cristina da Costa; Lima, Kenio Costa de; Freitas, Roseana de Almeida; Morais, Maria de Lourdes Silva Arruda de; Queiroz, Lelia Maria GuedesAntitumor immunity plays an important role in the development of and protection against malignancy. In general, patients with cancer are known to be immunologically compromised. The objective of this study was to evaluate local immunity in squamous cell carcinomas (SCCs) of the tongue and lower lip by immunohistochemistry, using anti-CD3, -CD4, -CD8, -CD25 and -zeta antibodies. Immunoexpression at the invasive front was compared considering anatomical tumor location and metastasis. The CD4/CD8 ratio was calculated for each case and associated with the variables. CD3+, CD4+, CD8+ and CD25+ cell counts were higher in SCCs of the lower lip and anti-zeta immunostaining was more evident in non-metastatic cases. CD8+ and CD25+ cell counts were also significantly correlated with tumor location (p=0.004 and p=0.004, respectively), with the observation of a larger number of these cells in SCCs of the lower lip. The CD4/CD8 ratio showed no significant association with metastasis or anatomical location. In conclusion, the clinical behavior of the oral SCC cases studied might be partially related to the immunohistochemical profile of the inflammatory infiltrate present at the invasive front.
