    Development of oral sensory-motor functions of preterm and low-birth-weight newborns under speech-language pathology care

    Introducción: Los recién nacidos (RN) prematuros y con bajo peso pueden presentar inmadurez en las funciones de succión, deglución y respiración. Los logopedas en los hospitales trabajan el desarrollo del sistema sensoriomotor oral de los RN, promoviendo una transición segura de la sonda a la lactancia materna, lo que contribuye a mejorar la calidad de vida de la población infantil. El objetivo del presente estudio era analizar el desarrollo de las funciones orales, el tiempo de transición a la alimentación por vía oral (VO) y la lactancia materna de los RN prematuros, y con bajo peso al nacer bajo el cuidado fonoaudiológico. Métodos Este estudio de pronóstico se llevó a cabo en una maternidad, con base en la recopilación de datos en prontuarios archivados de 121 RN atendidos entre septiembre de 2015 y julio de 2017. En el análisis de los datos se aplicó el método de Kaplan-Meier, el test log-rank y la prueba de correlación de Pearson. Se consideró un nivel de significancia de 0,05 (95%). Resultados: Se observó que cuanto menores eran la edad gestacional y el peso de nacimiento del RN, más necesidad de servicios fonoaudiológicos había hasta el establecimiento de la VO exclusiva, y el tiempo de transición y el tiempo promedio de utilización de la sonda orogástrica eran inversamente proporcionales a la edad gestacional al nacimiento. La técnica de succión no nutritiva fue la más utilizada para la estimulación, y el 78,5% de los RN recibieron el alta hospitalaria con lactancia exclusiva. Conclusión: Los RN prematuros de moderados a tardíos y de bajo peso son capaces de adquirir el patrón funcional del sistema sensoriomotor oral con mayor rapidez, y hay indicios de que la atención fonoaudiológica reduce el tiempo de transición alimentaria a la VO, y aumenta la tasa de éxito de lactancia materna exclusiva

    Frequência da deficiência auditiva relacionada as infecções congênitas: estudo transversal retrospectivo

    Introduction: Congenital infections during pregnancy are risk indicators for hearing loss. Purpose: To verify the frequency of hearing loss in children attended at the public service with risk indicators for congenital infections. Methods: This is a retrospective cross-sectional study. The population consisted of children aged 0 to 3 years attended in the period from 2011 to 2019. Consultation and analysis were carried out in the Institution’s database, extracting information from the children regarding the presence of reported congenital infection (cytomegalovirus, herpes, rubella, syphilis, toxoplasmosis, HIV and Zika virus) and the complete audiological diagnosis. The sample of this study consisted of 558 children and the presence of co-occurrence between infections or other risk indicators for hearing loss was analyzed. Descriptive analysis was performed to establish the frequency of hearing loss in relation to each congenital infection isolated or associated with other risk indicators. Results: 14.40% of the children had a report of isolated congenital infection or in combination with another risk indicator. The frequency of hearing loss was 1.25%, with sensorineural hearing loss in six children (85.71%) and a conductive hearing loss (14.29%), of which six were bilateral (85, 71%) and one unilateral (14.29%). This frequency of hearing loss was related to the history of cytomegalovirus (57.14%), followed by toxoplasmosis (28.57%) and rubella with zika virus (14.29%). Conclusion: The frequency of diagnosis of hearing loss was 1.25% in children with reports of congenital infections

    A comparative study on phonological acquisition and performance in phonological awareness by children exposed to a bilingual or monolingual family environment

    Objective: to verify and compare phonological acquisition and performance in phonological awareness tasks among children exposed to a bilingual (Brazilian Portuguese and German) or monolingual family environments (Brazilian Portuguese). Methods: a sample of 32 children of both sexes aged 5 years were divided into the bilingual group (13 children), who lived daily with people who spoke German and Portuguese, and the monolingual group (19 children), who lived daily with people who spoke only the Portuguese language. Phonological acquisition and phonological awareness skills were assessed using protocols standardized for Brazilian Portuguese and statistically analyzed using the Fisher exact test and Mann-Whitney test, considering p<0.05. Results: the results revealed that a greater number of subjects demonstrated atypical phonological acquisition in the bilingual group and scored worse results in the phonological awareness skills evaluation. Phonological processes were similar in both groups. None of the results obtained were statistically significant. Conclusion: deviant phonological acquisition was superior in the bilingual family environment group. Phonological processes were similar for both groups. In the phonological awareness tasks, children in the bilingual family environment group obtained better results only for the phonemic synthesis tasks. For other tasks, children in the monolingual family group achieved better results

    Desempenho em consciência fonológica e erros de escrita de crianças submetidas a diferentes métodos de alfabetização

    Purpose: to evaluate the phonological awareness and writing abilities of children exposed to different literacy methods. Methods: The sample consisted of 29 children (9 were literate by the Phonic Method and 20 were literate by the Syllabic Method), with ages between 7:0 and 8:0 years, without gender equation. The children were enrolled in the 2nd year of elementary school, and had attended previous classes in the same school, had not had any speech therapy intervention, and had no detectable cognitive, psychological or emotional changes. All the children were submitted to the phonological awareness assessment of the Phonological Awareness Test and the evaluation of the writing through the Orthographic Observation Guide. The hits and errors were tabulated considering the specifications of each of the tests. Subsequently the data were submitted to descriptive statistical analysis and Mann Whitney test. Results:There was no statistically significant difference in any of the aspects analyzed, although it was observed a better performance of the children of the Syllabic Group in most tasks of the phonological awareness test, except for the tests of syllabic synthesis and syllabic manipulation. In these two tests, there were more occurrences of errors and identical results, respectively. As for the results obtained in the writing evaluation, the groups presented similar results. Conclusion: in the present study, the Syllabic Group presented superior performance in most of the phonological awareness activities, and a lower average of orthographic errors per child in the evaluation of writing errors

    O uso de um software na (re)habilitação de crianças com deficiência auditiva

    PURPOSE: To verify the applicability of a software in the (re)habilitation of hearing impaired children. METHODS: The sample comprised 17 children with hearing impairment, ten with cochlear implants (CI) and seven with hearing aids (HA). The Software Auxiliar na Reabilitação de Distúrbios Auditivos - SARDA (Auxiliary Software for the Rehabilitation of Hearing Disorders) was used. The training protocol was applied for 30 minutes, twice a week, for the necessary time to complete the strategies proposed in the software. To measure the software's applicability for training the speech perception ability in quiet and in noise, subjects were assessed through the Hearing in Noise Test (HINT), before and after the auditory training. Data were statistically analyzed. RESULTS: The group of CI users needed, in average, 12.2 days to finish the strategies, and the group of HA users, in average 10.14 days. Both groups presented differences between pre and post assessments, both in quiet and in noise. Younger children showed more difficulty executing the strategies, however, there was no correlation between age and performance. The type of electronic device did not influence the training. Children presented greater difficulty in the strategy involving non-verbal stimuli and in the strategy with verbal stimuli that trains the sustained attention ability. Children's attention and motivation during stimulation were fundamental for a successful auditory training. CONCLUSION: The auditory training using the SARDA was effective, providing improvement of the speech perception ability, both in quiet and in noise, for the hearing impaired children.

    Study of the prevalence of impaired hearing and its determinants in the city of Itajaí, Santa Catarina State, Brazil

    The number of people with impaired hearing is increasing; knowing its magnitude is essential for public health. Objective: To study the prevalence and determinants of impaired hearing in Itajaí/ SC. Methods: A population-based survey based on a World Health Organization protocol. Field research was carried out from July 2008 to 2011. Procedures for evaluating hearing in households: questionnaire, measurement of noise, otoscopy, pure tone audiometry at 1000, 2000, and 4000 Hz, tympanometry, and acoustic reflexes: individuals above 4 years; children under 4 years: evoked otoacoustic emissions (OAE), cocleo-palpebral reflex(CPR), tympanometry, and acoustic reflexes. In the elderly population a questionnaire of perception of hearing loss was applied. Results: The study sample consisted of 379 individuals - 45.38% were males and 54.62% were females. Agerange: 11.34% up to 10 years; 64.39% 10 to 60 years, 24.27% over 60 years. Evaluation of the best hearing ear showed that 74.1% of residents had normal hearing, 18.9% had mild hearing loss, 5.1% had moderate hearing loss, 1.9% had severe hearing loss. Disabling impaired hearing was detected in 26 subjects: one child (otitis media); four adults (one otitis, one noise-induced, two idiopathic); 21 elderlies (presbyacusis). Of eight children under 4 years all presented CPR, three were normal examinations, two had absent OAE bilaterally, one had absent OAE in the right ear and one in the left ear. Conclusion: The prevalence of disabling impaired hearing in Itajaí was 7%; the highest prevalence was in the 50-year and above age group - the main cause was presbyacusis.

    Effectiveness of vocal therapy for the elderly when applying conventional and intensive approaches: a randomized clinical trial

    Objectives: The aim of this study was to verify the effects of the method Vocal Therapy for the Elderly and the differences in treatment efficacy when it was administered intensively or in the conventional way. Methods> Twenty-seven elderly individuals were randomized into two groups and referred for 16 sessions of vocal therapy. The Intensive Group (IG) had therapy four times a week, whereas the Conventional Group had it twice a week. The effects of the therapy were assessed by auditory-perceptual analysis, the Voice-Related Quality of Life protocol, and visual-perceptive analysis of laryngoscopy examinations. The first stage consisted of evaluating the vocal quality and self-assessment of 15 subjects before and after a time period equal to that which they would undergo in vocal therapy. The second stage consisted of comparing the assessments of all participants in the week preceding the beginning of treatment, in the week following the end of treatment, and 1 month after that. Results: There was no difference between perceptual voice parameters and self-assessment when the subjects were not undergoing therapy. When comparing the periods immediately before and after therapy, there was improvement in vocal quality and Voice-Related Quality of Life. One month later, the benefits that had been revealed through the self-assessment protocol, and some of the improvements in vocal parameters were maintained. There was no difference between the IG and Conventional Group with the exception of vocal fold bowing, which decreased in the IG group. Conclusions: The Vocal Therapy for the Elderly program is effective for treating voice presbyphonia. An intensive approach may be superior with regard to vocal fold bowing.

    Cerebral palsy: association between nutritional status and occurrence of oropharyngeal dysphagia

    Objetivo: verificar a associação entre estado nutricional e ocorrência de disfagia orofaríngea em indivíduos com paralisia cerebral. Métodos: estudo transversal, quantitativo, realizado com indivíduos entre dois e 20 anos. Foi avaliado comprometimento neuromotor, ocorrência e grau de disfagia, antropometria, consumo alimentar, via alimentar, fracionamento e consistência da dieta. A avaliação da deglutição foi realizada por fonoaudióloga capacitada, por meio do Protocolo para Avaliação Clínica da Disfagia Pediátrica. Foi realizada análise estatística pertinente, adotando p<0,05%. Resultados: participaram 40 indivíduos com mediana de 8,7 (5,45-14,5) anos, sendo 65% do sexo masculino. Quanto ao comprometimento neuromotor, 72,5% foram classificados no nível IV e V. A prevalência de disfagia foi de 70%, via alimentar predominantemente oral (77,5%) e consistência da dieta normal (55%). Nas curvas de crescimento, todos foram classificados com estatura adequada para idade. Para peso, índice de massa corporal e dobras cutâneas houve, respectivamente, 82,5%, 85% e 62,5% em eutrofia. Foi encontrada associação estatisticamente significante entre disfagia e índice de massa corporal (p=0,018). A média diária de ingestão calórica e proteica foi de 1427,29±338,62 kcal e 56,86±17,57 gramas, respectivamente. Foi encontrada associação estatisticamente significante entre via alimentar e quantidade de proteína ingerida diariamente (p=0,041). Conclusão: a maioria dos indivíduos apresentou disfagia orofaríngea e comprometimento motor mais grave, alimentavam-se por via oral com consistência de dieta normal. Observou-se que quanto maior a dificuldade de deglutição, maior o comprometimento do estado nutricional.

    Hearing screening in schoolchildren: accuracy of different criteria used to analyze transient evoked otoacoustic emissions

    Purpose: to compare the accuracy of different criteria used to analyze transient evoked otoacoustic emissions in schoolchildren. Methods: an accuracy study, where an audiological assessment (audiometry, logoaudiometry, tympanometry) and transient emissions were performed with 70 schoolchildren, from the first to the fifth grade of a municipal school, in Northeastern Brazil (6-14 years, 9.9 ± 2 years), with four criteria, all with signal-to-noise ratio ≥ 3 dB, being: criterion A, in all frequency bands; B, in three consecutive frequency bands; C, in three of the five non-consecutive frequency bands; D, in 2, 3 and 4 kHz. Sensitivity, specificity, accuracy and predictive values with their respective confidence intervals of 95% were analyzed. Results: criterion A showed higher sensitivity (92.31%, 95% CI: 67-98%) and lower specificity (17.35%, 95% CI: 10-29%); criterion C higher specificity (84.21%, 95% CI: 72-91%) and higher positive predictive value (52.63%; 95% CI: 51.63-54.63). Accuracy was 82.85% (95% CI 78.23-87.47) in criterion C and 70% (95% CI: 65.38-74.62) in criterion B. Conclusion: criterion C, signal-to-noise ratio ≥ 3dB in three non-consecutive frequency bands, showed the best accuracy, being considered the best choice as a criterion for the isolated use of transient emissions as a hearing screening procedure, in schoolchildren.

    Frequency modulation system and speech perception in the classroom: a systematic literature review

    PURPOSE: This review aimed at presenting the benefits regarding the speech perception in noise shown by children who wear hearing aid devices and/or cochlear implants with the Frequency Modulation (FM) System at school. RESEARCH STRATEGY: A bibliographic survey was conducted in an electronic database with standardized search until the year 2012, and a manual search was performed by using specific keywords. SELECTION CRITERIA: For the selection and evaluation of the scientific studies chosen in the search, criteria were established covering the following aspects: type of study, participants, adopted intervention, and evaluation of results. DATA ANALYSIS: The FM system was verified to improve speech perception and speech threshold in noise in all studies. RESULTS: Regarding the performance as to type, the best results were obtained when children used the personal FM system, followed by the table and the sound field systems. CONCLUSION: After extensive review of national and international literature, it was concluded that the studies indicate the need for further research concerning mainly the impact of the FM system on the school performance of children who have sensory devices coupled to the FM system. Findings in the literature with relation to the publications focused on speech perception in noise did not relate educational and auditory aspects.

    Influence of the socieconomic level on the temporal resolution hearing skills in adults

    Objective: to investigate the influence of the socioeconomic level on the temporal resolution auditory ability of adults. Methods: the sample consisted of 48 subjects aged 18 to 50 years, divided into three groups: G1, 11 subjects at level A; G2, 19 in B1 and B2 and G3, 18 in C1, C2, D and E. All subjects presented responses in 20 dB HL in frequencies of 500 to 4000 Hz, during audiometric screening, type A tympanometry, presence of contralateral and ipsilateral acoustic reflexes, no neurological, psychiatric and / or psychological changes diagnosed; without audiological and / or otological complaints and with performance above 95% in the dichotic digit test. The Random Gap Detection and Gap-in-noise tests were performed. The ShapiroWilk test for normality analysis and the Kruskall Wallis test for socioeconomic stratum analysis, were used, both with 5% significance. Results: there were statistically significant differences between the groups in the Random Gap Detection in the frequency of 500 Hz and in the average of the frequencies, as a function of the socioeconomic level, the same not being observed in the Gap-in-noise. Conclusions: it is suggested that the socioeconomic level be taken into account in the analysis of the Random Gap Detection test

    Accuracy of smartphone-based hearing screening tests: a systematic review

    Purpose: To verify the accuracy of smartphone apps to identify hearing loss. Research strategies: A systematic review followed the PRISMA-DATA checklist. The search strategies were applied across four databases (Lilacs, PubMed, Scopus and Web of Science) and grey literature (Google Scholar, OpenGrey, and ProQuest Dissertations and Thesis). Selection criteria: The acronym PIRD was used in review. This included populations of any gender and all age groups. The Index test is the smartphone-based hearing screening test; the Reference test is the puretone audiometry, which is considered the gold reference for hearing diagnostics; the diagnosis was performed via validity data (sensitivity and specificity) to identify hearing loss and diagnostic studies. Data analysis: Two reviewers selected the studies in a two-step process. The risk of bias was assessed according to the criteria of the QUADAS-2. Results: Of 1395 articles, 104 articles were eligible for full-text reading and 17 were included. Only four met all criteria for methodological quality. All of the included studies were published in English between 2015 and 2020. The applications Digits-in noise Test (5 articles), uHear (4 articles), HearScreen (2 articles), hearTest (2 articles) and Hearing Test (2 articles) were the most studied. All this application showed sensitivity and specificity values between 75 and 100%. The other applications were EarScale, uHearing Test, Free field hearing (FFH) and Free Hearing Test. Conclusion: uHear, Digit-in-Noise Test, HearTest and HearScreen have shown significant values of sensitivity and specificity and can be considered as the most accurate methods for screening of hearing impairment.
